Abstract : Introduction : Population ageing is associated with an increase in polypharmacy, exposing older patients to a higer risk of advser drug events. Continuity of therapeutic decisions between hospital care and primary care largely depends on the quality of the hospital discharge summary. Materials and Methods : This single centre observational study was conducted in a genereal internal medicine department and included patients aged 75 years and older who experienced at least ine modification od their chronic treatment during hospitalization. The primary objective was to compare the number of unexpected medication changes made by the primary care physician after hospital discharge according to the presence of treatment justification in the discharge summary. Results : A total of 89 patients were analysed. Hospitalisation was characterized by numerous therapeutic adjustements. The rate of unexpected post-discharge medication changes was similar whether the treatment justification was located in the designated section of the discharge summary or elsewhere. However, a complementary analysis suggested a trend towards fewer unexpected modifications when the justification was provided in the designated section. Discussion : These findings highlight the importance of clarity and readability of the hospital discharge summary in order to optimize continuity of care for older patients with polypharmacy.
