Dans cette indication, peu d'études se sont intéressées au versant glénoïdien de l'implant, implanté sur une scapula généralement saine. Hypothèse : Les implants glénoïdiens des PTEI traumatiques sont bien positionnés et peu sujets aux complications. Matériels et Méthode : L'étude s'intègre au Symposium 2016 de la SOFCOT sur le Résultats des PTEI dans les fractures récentes de l’ESH. 513 patients avaient des données concernant l’implant glénoïdien et son suivi. L'analyse radiologique a évalué la position en hauteur et l'inclinaison de l'implant, ainsi que l'apparition d'une encoche ou d’un descellement. Le retentissement clinique a été mesuré par le score de Constant. Résultats : Une encoche est apparue chez 43,6% des patients pendant le suivi dont 7% d'encoches stade 3-4. Ce taux augmentait en cas de position glénoïdienne haute (62,5% vs 42,3%, p=0,03) ou d’orientation supérieure de la sphère (58,3% vs 37,8%, p=0,02). Neuf descellements ont été constatés (soit 1,75%). Ils étaient plus fréquents en cas d'orientation supérieure (9,3% vs 0,4%, p<0,001). La présence d'une encoche n'était pas corrélée au score de Constant. En revanche, une glène en position haute ou intermédiaire était corrélée à un score de Constant moins élevé (57 versus 52 et 45, p<0,001). Soixante-douze patients ont pu être suivis dans le temps. 59,7% présentaient une encoche au dernier recul dont 44,4% d'encoches qui se sont aggravées au cours du suivi. Discussion : L'étude du versant glénoïdien des PTEI traumatiques montre que l’implant glénoïdien est en général bien posé. En revanche, les implants trop hauts ou avec une orientation supérieure exposent à un risque accru de complications. Radiological analysis assessed the height and inclination of the implant, as well as the appearance of a notch or loosening. The clinical impact was measured by the Constant score. Results : A notch was seen in 43.6% of patients during follow-up, of which 7% were 3-4 grade. This rate increased in case of a high glenoid position (62.5% vs 42.3%, p = 0.03) or higher inclination of the glenosphere (58.3% vs. 37.8%, p = 0.02 ). Nine loosening were reported (1.75%). They were more frequent in cases of higher inclination (9.3% vs 0.4%, p <0.001). The presence of a notch was not correlated with the Constant score. A high or intermediate glenoid was correlated with a lower Constant score (57 versus 52 and 45, p <0.001). Seventy-two patients were followed over the time. 59.7% had a notch at the last recess, 44.4% of which had worsened during follow-up. Discussion : The study of the glenoid in the traumatic RSA shows that the glenoid implant is generally well implanted. On the other hand, implants that are too high or with a higher orientation expose them to an increased risk of complications.
