Etude : Etude rétrospective Méthode : L’étude incluait 58 patients entre Janvier 2015 et Avril 2017, opérés d’une chirurgie parotidienne partielle ou totale. La chirurgie ambulatoire a été proposée par deux chirurgiens entrainés à tous les patients sauf ceux ayant un trouble de l’hémostase. Il y avait 40 patients dans le groupe hospitalisation et 18 patients dans le groupe ambulatoire. La durée moyenne de suivi était de 14.9± 7.5 mois. Les données médicales ainsi que les complications ont été comparées entre les 2 groupes en réalisant des analyses univariées et multivariées. Résultats : Dans le groupe ambulatoire, 3 patients sont sortis après une nuit de surveillance, un pour des douleurs thoraciques et deux pour une sortie de salle tardive. Il y a eu des infections pour 4 patients nécessitant une ré hospitalisation (à 5 jours) dans le groupe ambulatoire, et pour 7 patients, nécessitant une ré hospitalisation pour 2 patients (à 5.5 jours) dans le groupe hospitalisé. Dans le groupe ambulatoire, seulement cinq patients ont eu un drainage postopératoire et le drain a été systématiquement enlevé avant la sortie du patient. En analyse multivariée, il n’y avait pas de différence de complications entre les deux groupes (p= 0.2071). Conclusion : La chirurgie parotidienne en ambulatoire a été possible pour 84% des patients initialement prévus en ambulatoire. L’ambulatoire n’était pas associé à un risque plus élevé de complications. Objective: The aim of this study was to compare follow up of patient treated in outpatient condition and inpatient condition for parotidectomy. Study design: retrospective chart view Methods: The study included 58 patients between January 2015 and April 2017. All patients underwent a partial or total parotidectomy. Outpatient surgery was proposed to all patients but those with coagulopathy and performed by two high-volume surgeons. There were 40 patients in the inpatient group, 18 in the outpatient group. Median follow up time was 14.9± 7.5 months. Characteristic’s patients, clinical data and complications were compared among groups using univariate and multivariate analysis. Results: In outpatient group, three patients were discharged after one night follow-up, one for postoperative chest pain, two for later surgery. Abscesses occurred in four patients and always required rehospitalization (day 5) in the outpatient group and in seven patients with rehospitalization in two (day 5.5) in the inpatient group. In outpatient group, only five patients had postoperative drainage and drain was always removed before discharge. In multivariate analysis, there was no difference of overall complications (p= 0.2071), salivary fistula (p= 0.3348), abscesses (p= 0.2110), facial nerve palsy (p= 0.4882) between outpatient and inpatient. Conclusion: Outpatient parotid surgery was feasible in 84% of patient initially planned. Outpatient condition was not associated to a higher risk of complications.
