     <dcterms:abstract xml:lang="fr">Introduction : Plusieurs études internationales ont démontré l’intérêt des soins palliatifs débutés précocement dans le parcours de soin des patients. En France, cette démarche devient un enjeu de santé publique. L’HAS propose plusieurs outils pour y parvenir dont la question surprise (SQ). Nous avons étudié l’appropriation de cet outil par les médecins généralistes de 3 communes d’Ille-et-Vilaine. Matériel et méthodes : Il s’agit d’une étude prospective mixte qualitative et quantitative qui a été menée auprès de 14 praticiens durant 18 mois. Pendant 6 mois d’inclusion, ils ont repéré des patients nécessitant des soins palliatifs en utilisant la question surprise. Le suivi a duré 1 an : les généralistes nous transmettaient la survenue des décès. Les médecins ont été interviewés à deux reprises (6 et 18 mois) via des entretiens semi dirigés. Résultats : 87 patients ont été inclus. 59% sont décédés dans les 12 mois du suivi. La sensibilité calculée était de 60%. A 6 mois : les médecins ont souligné la simplicité de l’outil. La SQ leur a permis de s’interroger, d’anticiper des situations et d’être plus attentif aux prises en charge. A 18 mois : ils confirment la simplicité et conseillent la SQ. Elle influence leur prise en charge et modifie la relation qu’ils construisent avec le patient. Discussion : Les médecins se sont saisis de la question surprise. La population paraît être représentative de la pratique générale. Nos données statistiques coïncident avec ceux de la littérature. La SQ remplit les critères décrits d’un bon outil de repérage et apparaît donc comme un levier à la mesure 12 du plan national. Conclusion : La SQ est facilement utilisable en soins primaires, elle est appréciée des praticiens et ces derniers se l’approprient assez naturellement. Il est délicat d’affirmer que cela a changé leur pratique, à défaut, on peut convenir que la question modifie le regard porté sur le patient et invite le praticien à mobiliser d’autres ressources pour le prendre en charge.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">Introduction: Several international studies demonstrated the benefit of early palliative care. France has proposed a new public health approach to palliative care. French high authority of health recommends tools to identify patient who need palliative care including the surprise question. We studied the appropriation of this tool by the general practitioners (Gps). Methods: It is a qualitative and quantitative study realized with 14 GPs of Ille-&amp;-Villaine (Britany) during 18 months. They included during 6 months patients requiring palliative care by using the surprise question. Follow-up of the death lasted 1 year. Semi directed interviews were conducted at the 6th and the 18th month.  Results: 87 patients were included. 59% died in 12 months. Sensibility was 60%. In the interview of the 6th month, GPs noticed the simplicity of the tool. SQ allows them to wonder, to anticipate situations, and be more attentive. In the interview of the 18 month, they confirm the simplicity and recommend it. SQ influences their treatments and changes the relation between patient and doctor. Discussion: The GPs adopted the tool. Population study seems to correspond with a standard practice. Statistics results obtained are in good agreement with the data from the literature. The surprise question is a good tool to identify patients who need palliative care and seems to be a good response to the 12th measure of the French national plan for the development of the palliative care. Conclusion: SQ is easily usable in primary care. Practitioners appreciated the tool and appropriate it. SQ change the way the GPs look the patient and can use other capacities to take care of him.</dcterms:abstract>
