L’objectif de notre étude est de décrire la symptomatologie des intoxications au baclofène en réanimation. Méthodes : Dans cette étude rétrospective, nous avons inclus tous les patients hospitalisés en réanimation pour intoxication aigue au baclofène. Résultats : Entre juin 2013 et janvier 2016, 20 patients (65% d’hommes) ont présenté une intoxication au baclofène avec un âge moyen de 44,3 ans. Ils étaient tous traités par baclofène pour sevrage alcoolique. Il n’y a pas eu de décès. Les durées médianes d’hospitalisation et de ventilation étaient respectivement 4 jours et 44 heures. Il s’agissait d’une mono-intoxication pour 5 patients. La quantité supposée ingérée moyenne était de 746 mg et la baclofènémie médiane de 2,65 mg/l. Les signes neurologiques étaient au premier plan avec un coma (Glasgow moyen à 5) associé à des phases d’agitation (n=13) et des mouvements anormaux (convulsions tonico-cloniques, myoclonies ou mouvements désorganisés) chez 9 patients. Les réflexes ostéo-tendineux étaient absents chez 8 patients et 11 patients avaient un myosis. Les EEG étaient anormaux avec 2 tracés de burst suppression, 4 tracés d’encéphalopathie avec ondes triphasiques et un tracé lent. Les autres signes cliniques étaient des hypotensions (n=6), bradycardies (n=4), pauses respiratoires (n=8), insuffisances rénales aigues (n=4), hypoglycémies (n=3), vomissements (n=6) et hypothermie (n=5). Conclusion : Les intoxications aigues au baclofène en réanimation sont dominées par les signes neurologiques qui associent coma calme et agitation. Les reconnaître facilite le diagnostic. Introduction : Baclofen intoxications are poorly described and are increasing since the use of baclofen for alcohol withdrawal. The aim of the study was to describe the clinical symptoms of baclofen intoxications in intensive care unit. Méthods : In this retrospective study, we included all patients hospitalized in intensive care for acute intoxication baclofen. Results : Between June 2013 and January 2016, 20 patients (65% men) had baclofen intoxications with a mean age of 44,3 years. They were all treated by baclofen for alcohol withdrawal. There was no death. The median length of hospitalization and ventilation were respectively 4 days and 44 hours. It was a mono-intoxication for 5 patients. The mean supposed ingested dose of baclofen was 746 mg and the median serum concentrations of baclofen was 2,65 mg/l. Neurological signs were in the foreground with coma (mean Glasgow coma scale = 5) associated with agitation (n=13) and abnormal movements (seizures, myoclonus and disorganized movements) for 9 patients. Deep tendon reflexes were absent in 8 patients and 11 patients had myosis. Electroencephalograms (EEG) were abnormal with 2 suppression burst, 4 encephalopathy patterns with triphasic waves and a slow EEG. Others signs were hypotension (n=6), bradycardia (n=4), respiratory pauses (n=8), acute renal failure (n=4), hypoglycemia (n=3), vomiting (n=6) et hypothermia (n=5). Conclusion : acute baclofen intoxications in intensive care unit are dominated by neurological signs that combine calm coma and agitation. Recognize them facilate the diagnosis.
