Sa prévalence en fait une problématique majeure dans la prise en charge mais ses fondements psychopathologiques sont mal connus. Les données de la littérature semblent en faveur d’une aggravation au cours du temps de l’apathie dans la schizophrénie. L’association de l’apathie à l’altération des fonctions exécutives est retrouvée dans des données récentes. La capacité d’inhibition cognitive, composante des fonctions exécutives, semble altérée dans la schizophrénie mais les résultats restent hétérogènes. L’objectif de notre étude était d’étudier l’évolution de l’apathie chez les patients schizophrènes chroniques stables et le lien avec l’altération de la capacité d’inhibition cognitive. Méthode: Nous avons évalué l’apathie (AESc) et la capacité d’inhibition cognitive (TMT et test de Stroop) de façon prospective chez 18 patients schizophrènes stables. Résultats: Nous avons observé une augmentation des scores à l’AESc entre t0 et t1 (ΔAESc = 7,8 (σ = 18,8)). Nous avons observé une corrélation significative négative entre la variation du niveau d’apathie et la variation du score TMT B-A entre t0 et t1 (r = -0.50, p = 0,03). Nous avons observé une corrélation significative positive après ajustement sur l’âge et les équivalents Chlorpromazine (r = 0.57, p = 0,01). Nous n’avons pas trouvé de corrélation significative entre la variation du niveau d’apathie et la variation du score d’interférence au Stroop entre t0 et t1, y compris après ajustement sur l’âge et les équivalents Chlorpromazine. Conclusion: Notre étude a suggéré un lien entre apathie et l'altération de la capacité d'inhibition dans la schizophrénie. Introduction: Apathy is a common negative syndrome in schizophrenia which is associated with impaired functional outcome. However its psychopathological foundations remain largely unknown. Data from international literature seems to evidence a worsening over time of apathy in schizophrenia. Lately, an association between apathy and impaired executive functions has been found. Cognitive inhibition is a component of executive functions. It seems impaired in schizophrenia but the data varies. The aim of our study was to measure the evolution of apathy in stable patients with schizophrenia and to investigate an association with cognitive inhibition ability impairment. Method: We evaluated apathy (AESc) and the cognitive inhibition ability (TMT and Stroop test) prospectively in 18 stable patients with schizophrenia. Results: We observed an increase in scores at AESc between t0 and t1 (ΔAESc = 7.8 (σ = 18.8)). There was a significant negative correlation between changes in the level of apathy and the variation of the TMT B-A score between t0 and t1 (r = -0.50, p = 0.03). There was a significant positive correlation after adjustment for age and chlorpromazine equivalents (r = 0.57, p = 0.01). There was no significant correlation between changes in the level of apathy and the variation of the Stroop interference score between t0 and t1, even after adjustment for age and chlorpromazine equivalents. Conclusion: Our study suggested a link between apathy and alteration of cognitive inhibition ability in schizophrenia.
