     <dcterms:abstract xml:lang="fr">Introduction – Le cancer du sein triple-négatif (CSTN) est un sous-type moléculaire moins fréquent mais plus agressif de cancer du sein que l’on retrouve notamment chez des patientes plus jeunes. S’il ne possède pas de cible thérapeutique spécifique, la découverte de sa forte immunogénicité a permis le développement de nouvelles thérapeutiques sous forme d’immunothérapie, y compris en situation néoadjuvante. Néanmoins, pour mieux comprendre et prédire la réponse aux traitements, une meilleure caractérisation immune est nécessaire. Méthodes – Après une caractérisation de 91 CSTN par cytométrie en flux (CMF), 19 de ces tumeurs ont été caractérisées par IHC multiplexe par technique Hyperion et 25 par analyse transcriptomique. En parallèle, un modèle murin a été développé tout comme une étude radiomique prospective. Résultats – Après caractérisation des sous-populations immunes par CMF, seuls les lymphocytes CD4+ (LT CD4+) Th2 avaient une valeur pronostique pour la survie sans récidive et globale, notamment plus forte que les TILs. Les tentatives de validations orthogonales ont permis de retrouver des valeurs de leucocytes CD45+, LT CD4+, voire CD 8+, corrélées en IHC et transcriptomique mais ont manqué de puissance pour que cela puisse être le cas pour les Th2. Par ailleurs, pour une meilleure corrélation aux schémas néoadjuvants actuels, un modèle murin a été créé permettant l’étude des sous-populations immunes et une analyse radiomique a été menée sur une cohorte prospective. Conclusion – Les LT CD4+ Th2 mesurés par CMF ont une valeur pronostique en adjuvant mais celle-ci, tout comme celle prédictive, restent à prouver en néoadjuvant par différentes techniques utilisables en pratique clinique.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">Introduction – Triple-negative breast cancer (TNBC) is a less common but more aggressive molecular subtype of breast cancer that is found particularly in younger patients. Although it has no specific therapeutic target, the discovery of its high immunogenicity has led to the development of new immunotherapy treatments, including in neoadjuvant settings. However, to better understand and predict response to treatment, more detailed immune characterization is needed. Methods – After characterizing 91 TNBCs by flow cytometry (FC), 19 of these tumors were characterized by multiplex IHC using Hyperion technique, and 25 by transcriptomic analysis. In parallel, a mouse model was developed, as was a prospective radiomic study. Results – After characterization of immune subpopulations by FC, only Th2 CD4+ T cells (CD4+ TC) had a prognostic value for recurrence free and overall survival, particularly stronger than TILs. Orthogonal validation attempts found values for CD45+ leukocytes, CD4+TCs, and even CD8+TCs that correlated in IHC and transcriptomics, but lacked the power to do so for Th2 cells. Besides, to improve correlation with current neoadjuvant regimens, a mouse model was created to study immune subpopulations, and a radiomic analysis was performed on a prospective cohort. Conclusion – Th2 CD4+ TCs measured by FC have a prognostic value in adjuvant therapy settings, but this value, like their predictive one, remains to be proven in neoadjuvant settings by various techniques that can be used in clinical practice.</dcterms:abstract>
