La contribution génétique apparaît majeure dans l’IOP, bien que les mécanismes moléculaires impliqués ne restent que partiellement élucidés. L’objectif de cette thèse est d’explorer la complexité de l’architecture génique de l’IOP. Dans un premier volet, ce travail identifie SWSAP1 comme un nouveau gène d’IOP. L’analyse fonctionnelle, réalisée par le test de recombinaison homologue IH-HR, a permis de valider l’impact pathogène de variants retrouvés dans SWSAP1 et ZSWIM7, deux gènes participant au complexe Shu, structure essentielle à la recombinaison homologue méiotique. Dans un second temps, cette thèse met en évidence l’importance des variations du nombre de copies au sein du gène FSHR, en soulignant la diversité des altérations génétiques. Enfin, l’étude d’une cohorte de patientes porteuses de réarrangements chromosomiques apparemment équilibrés a permis d’identifier des gènes candidats localisés aux points de cassure, susceptibles de jouer un rôle clé dans la physiopathologie de l’infertilité féminine. Dans son ensemble, ce travail illustre la richesse et la complexité de l’architecture génique de l’IOP, en combinant des approches de cytogénétique, de génomique et de validation fonctionnelle. Il contribue à affiner les stratégies de diagnostic génétique, à élargir le spectre des gènes impliqués dans la physiopathologie ovarienne et à ouvrir la voie à une prise en charge plus personnalisée des patientes. Premature ovarian insufficiency (POI) is a heterogeneous disorder characterized by the cessation of ovarian function before the age of 40. Genetic contribution appears to play a major role in POI, although the underlying molecular mechanisms remain only partially understood. The objective of this thesis is to explore the complexity of the genetic architecture of POI. In the first part, this work identifies SWSAP1 as a novel POI gene. Functional analyses, including the IH-HR homologous recombination assay, confirmed the pathogenic impact of variants identified in SWSAP1 and ZSWIM7, two genes involved in the Shu complex, an essential structure for meiotic homologous recombination. In the second part, this thesis highlights the importance of copy number variations within the FSHR gene, emphasizing the diversity of genetic alterations identified. Finally, the study of a cohort of patients carrying apparently balanced chromosomal rearrangements enabled the identification of candidate genes located at the breakpoint regions, which may play a key role in the pathophysiology of female infertility. Overall, this work illustrates the richness and complexity of the genetic architecture of POI, combining cytogenetic, genomic, and functional approaches. It contributes to refining genetic diagnostic strategies, broadening the spectrum of genes implicated in ovarian physiology, and paving the way toward more personalized care for patients.
