Cette étude vise à évaluer l’impact de la mise en place d’une application mobile (MyCHU Rennes®) dans la déclaration des réactions retardées après un séjour en HDJ d’allergologie et d’évaluer la sévérité des réactions déclarées. Méthodes : E-DREAM est une étude observationnelle comparative rétrospective monocentrique sur 384 séjours de patients majeurs après une réintroduction médicamenteuse ou une ITS au venin d’hyménoptère avant et après la mise en place d’une application en HDJ d’allergologie. Les caractéristiques de la population, des réactions initiales, des séjours ainsi que des réactions retardées ont été décrites. Résultats : Le nombre de déclarations de réaction retardée a presque doublé entre les deux périodes (passant de 3,3 % à 6,3 %) sans être statistiquement significatif. Quelques réactions retardées systémiques sévères sont notées sans différence significative entre les deux périodes, sans aucune toxidermie sévère déclarée. L’adhésion à l’utilisation de l’application a été estimée à environ 50 %. Il s’agit du moyen de déclaration principal : 81,8 % des patients déclarant une réaction l’ont utilisée. Conclusion : Les outils de santé-mobile permettent un meilleur recueil de l’information en sortie d’hospitalisation en limitant le recours au système de soin. Il s’agit du moyen de communication privilégié lors de la déclaration de réaction retardée. Il pourrait être étendu à d’autres explorations allergologiques dont les tests cutanés à lecture retardée, où l’aide d’une IA pourrait se discuter. Introduction: Delayed reactions occurring at home after a stay in an allergy day hospital are poorly characterized because they are underreported, but they can be potentially severe. This study aims to evaluate the impact of implementing a mobile application (MyCHU Rennes®) on the reporting of delayed reactions after a stay in an allergy day hospital and to assess the severity of the reported reactions. Methods: E-DREAM is a single-center, retrospective, comparative observational study of 384 stays by adult patients after drug challenge test or ITS with hymenoptera venom before and after the implementation of an application in the allergy day hospital. The characteristics of the population, the initial reactions, the hospital stays, and delayed reactions were described. Results: The number of delayed reaction reports almost doubled between the two periods (from 3.3% to 6.3%) without being statistically significant. A few severe systemic delayed reactions were noted with no significant difference between the two periods, and no severe toxidermia was reported. Adherence to the use of the app was estimated at around 50%. It is the primary means of reporting: 81.8% of patients reporting a reaction used it. Conclusion: Mobile health tools enable better collection of information upon discharge from hospital by limiting the use of the healthcare system. It is the preferred means of communication when reporting delayed reactions. It could be extended to other allergy tests, such as delayed-reading skin tests, where the use of AI could be discussed.
