Nous avons conduit une étude chez les patients opérés pour tumeur(s) surrénalienne(s) au CHU de Rennes afin d'évaluer la pertinence et de la sécurité de ces nouveaux critères. METHODES : 390 patients ont été inclus dans cette étude monocentrique, observationnelle et rétrospective. Les objectifs étaient d'analyser les caractéristiques radiologiques, hormonales et chirurgicales des pathologies représentées et d'évaluer la concordance entre l'estimation du risque de malignité basé sur l'imagerie et le diagnostic final. RESULTATS : Le phéochromocytome était le premier type tumoral opéré, suivi des métastases surrénaliennes. Les marges d'exérèses incomplètes étaient statistiquement plus fréquentes en chirurgie robot-assistée que pour d'autres techniques chirurgicales. La stratification du risque de malignité basée sur l’imagerie permettait d'écarter une tumeur maligne avec une excellente sensibilité de 99% et une VPN de 97%, mais peinait à distinguer les tumeurs bénignes pauvres en graisses, des tumeurs malignes (spécificité 15% ; VPP 63%). CONCLUSION : La sensibilité de la stratification du risque de malignité selon les recommandations ESE/ENSAT est excellente et permet d'éliminer une tumeur maligne de façon quasiment certaine. Toutefois, de nouveaux outils morphologiques et biologiques sont nécessaires pour identifier les adénomes bénins. La performance et la sécurité de la chirurgie robot-assistée versus gold standard doit être évaluée dans de plus larges cohortes. INTRODUCTION: Updated guidelines on the management of adrenal tumours have been published by the ESE/ENSAT in 2023, aiming to better identify which ones are secreting and/or are suspected to be malignant and require surgery. We conducted a study of patients who underwent resection of one or more adrenal tumours at Rennes University Hospital, to evaluate the relevance and safety of these new criteria. METHODS: This single-centre, observational, retrospective study included 390 patients. The objectives were to analyse the radiological, hormonal and surgical characteristics of the represented pathologies and to evaluate the concordance between the assessment of the risk of malignancy based on imaging and the final diagnosis. RESULTS: The most common diagnosis was pheochromocytoma, followed by adrenal metastases. Incomplete excision margins were statistically more frequent in robot-assisted surgery than in other approaches. The stratification of the risk of malignancy based on imaging permitted the exclusion of malignant tumours with an excellent sensitivity of 99% and a NPV of 97%, but failed to distinguish between benign, low fat, tumours, and cancers (specificity 15%; PPV 63%). CONCLUSION: The assessment of the risk of malignancy is highly sensitive, enabling malignant tumours to be eliminated with almost complete certainty. However, new morphological and biological tools are needed to identify benign adenomas. The performance and safety of robot-assisted surgery versus the gold standard must be evaluated in larger cohorts.
