     <dcterms:abstract xml:lang="fr">Contexte : Le Foramen Ovale Perméable (FOP) est une anomalie cardiaque impliquée dans la survenue des AVC ischémiques cryptogéniques, surtout chez les patients jeunes. La procédure de fermeture de FOP a prouvé son efficacité pour diminuer le risque de récidive ischémique depuis plusieurs années. Une sélection adaptée des patients doit avoir lieu, au cours de réunions de concertation pluridisciplinaires (RCP) neuro-cardiologiques. Objectifs : Etudier le devenir des patients présentés pour discussion de fermeture de FOP en RCP neuro-cardiologique au CHU de Rennes, en évaluant le taux de récidive d’ischémie cérébrale à 2 ans. Le but final étant l’amélioration des pratiques cliniques du CHU de Rennes. Méthodes : Etude observationnelle rétrospective incluant les patients présentés en RCP entre 2018 et 2021. Les données cliniques et paracliniques ainsi que celles du suivi ont été recueillies à partir des dossiers médicaux et grâce à un questionnaire téléphonique. Le critère de jugement principal était la survenue d’une récidive ischémique à 2 ans de la présentation en RCP. Les critères de jugements secondaires étaient : la description des patients ayant présenté une récidive, l’évaluation du taux de fermeture de FOP, des complications post-procédure, de la qualité du suivi et du handicap résiduel à long terme. Résultats : Au total, 193 patients ont été inclus, d’âge moyen 46,5 ans, avec une majorité d’hommes. A 2 ans de la RCP, 5 patients (2,6%) ont présenté une récidive : 3 (2,3%) dans le groupe fermé et 2 (3,2%) dans le groupe non fermé, sans différence significative (p=0,6570). A plus long terme, il semble y avoir plus de récidives dans le groupe non fermé (sans significativité). Les récidives du groupe fermé ont eu lieu entre la RCP et la fermeture. Le taux réel de fermeture de FOP est de 60%. Le taux de complication est de 5% incluant 3,8% de passage en FA. Le suivi des patients respecte les recommandations internationales et le handicap résiduel est faible dans les deux groupes. Conclusion : Cette étude souligne le rôle essentiel de la RCP neuro-cardiologique dans la sélection des patients avant une procédure de fermeture de FOP. Le CHU de Rennes applique les bonnes pratiques cliniques avec un faible taux de récidive ischémique et de complications. Une réduction du délai entre l’évènement ischémique et la fermeture pourrait améliorer les résultats.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">Background: Patent Foramen Ovale (PFO) is a cardiac anomaly involved in cryptogenic ischemic strokes, particularly in young patients. Percutaneous closure has proven its effectiveness in reducing the risk of ischemic recurrence for several years. Careful patient selection is required and is usually carried out during multidisciplinary neuro-cardiology meetings (RCP). Objectives: To study the outcomes of patients presented for discussion of PFO closure in neuro-cardiology RCP at CHU de Rennes, by evaluating the rate of cerebral ischemic recurrence at 2 years. The ultimate goal is the improvement of clinical practices at CHU de Rennes. Methods: We conducted a retrospective observational study including patients presented at the RCP between 2018 and 2021. Clinical, paraclinical and follow-up data were collected from medical records and through a telephone survey. The primary endpoint was ischemic recurrence within 2 years of RCP presentation. Secondary endpoints included the description of patients with recurrence, the closure rate, post-procedural complications, quality of follow-up, and long- term residual disability. Results: A total of 193 patients were included, with a mean age of 46.5 years and a male predominance. At 2 years, ischemic recurrence occurred for 5 patients (2.6%): 3 (2.3%) in the closure group and 2 (3.2%) in the non-closure group, with no significant difference (p=0.6570). Over longer follow-up, a higher recurrence rate was observed in the non-closure group, although not significant. All recurrences in the closure group occurred between the RCP and the closure procedure. The actual closure rate was 60%. The overall complication rate was 5%, including 3.8% of new-onset atrial fibrillation. Patient follow-up was consistent with international recommendations, and residual disability was low in both groups. Conclusion: This study highlights the essential role of the neuro-cardiological RCP in selecting patients for PFO closure. CHU de Rennes applies good clinical practice, with low rates of ischemic recurrence and complications. Reducing the delay between the index stroke and closure could improve outcomes.</dcterms:abstract>
