Ce parcours, intégré aux soins courants, associe une évaluation clinique régulière à l’aide d’outils standardisés (MADRS, PHQ-9, FIBSER), un suivi thérapeutique pharmacologique et des tests pharmacogénétiques ciblant notamment les isoenzymes du cytochrome P450 et la P-glycoprotéine. Chaque inclusion donne systématiquement lieu à une réunion de concertation pluridisciplinaire, afin de croiser les expertises et de définir une stratégie thérapeutique individualisée. L’objectif d’OPTIDEP est de mesurer l’efficience et la pertinence de ce parcours en identifiant les outils indispensables ainsi que leur évolution après un an de suivi. OPTIDEP permet aussi de déterminer les outils utiles ainsi que les étapes réellement essentielles au bon déroulement du parcours voir à la rémission des patients. Ainsi, OPTIDEP permet d’améliorer en continu le parcours de soins des patients atteints de dépression résistante et d’orienter ce parcours vers une psychiatrie plus personnalisée et plus précise. The OPTIDEP protocol evaluates a structured and personalized care pathway for patients with treatment-resistant depression. This pathway, integrated into routine care, combines regular clinical assessments using standardized tools (MADRS, PHQ-9, FIBSER), therapeutic drug monitoring, and pharmacogenetic testing targeting, in particular, cytochrome P450 isoenzymes and P-glycoprotein. Each inclusion systematically involves a multidisciplinary case conference to bring together expertise and define an individualized therapeutic strategy. The aim of OPTIDEP is to assess the efficiency and relevance of this care pathway by identifying the essential tools and evaluating their evolution after one year of follow-up. OPTIDEP also helps to determine which tools and steps of this pathway are critical to the proper functioning of the pathway and potentially to the patient remission. Therefore, OPTIDEP enables the continuous improvement of the care pathway for patients with treatment-resistant depression and guides it towards a more personalized and precise psychiatry.
