     <dcterms:abstract xml:lang="fr">La radiothérapie externe vise à administrer des doses élevées de rayonnements ionisants de manière ciblée aux tissus tumoraux, tout en épargnant au maximum les tissus sains environnants. Le workflow conventionnel repose sur l'imagerie scanner (CT), dont le faible contraste des tissus mous entraîne des incertitudes dans la délimitation des volumes cibles. L’IRM, grâce à sa meilleure résolution des tissus mous, constitue une alternative prometteuse. Toutefois, l’IRM ne fournit ni les informations sur la densité électroniques, élément indipensable au calcul de dose. Cette thèse développe des méthodes avancées de traitement d’image pour améliorer la radiothérapie adaptative guidée par IRM, en particulier via la génération d’images CT synthétiques (sCT) à partir d’IRM. Pour améliorer la généralisation des modèles, la synthèse s’appuie sur des paires IRM/CT provenant de plusieurs centres. Un recalage non rigide innovant, utilisant une sCT intermédiaire, a également été conçu pour améliorer l’alignement entre IRM et CT. L’ensemble repose sur un réseau de neurones non supervisé. Par ailleurs, l’optimisation du workflow en IRM-linac permet une adaptation du traitement en temps réel grâce à l’acquisition quotidienne d’IRM. Cela permet l’estimation dynamique et le cumul des doses délivrées, guidant les décisions cliniques. En somme, cette recherche contribue à renforcer la précision et la fiabilité de la radiothérapie adaptative guidée par IRM, dans le but d’améliorer les résultats cliniques pour les patients.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">External beam radiotherapy aims to deliver high doses of ionising radiation precisely to tumour tissues while sparing surrounding healthy tissue as much as possible. The conventional workflow relies on CT imaging, which offers poor soft tissue contrast and leads to uncertainties in target volume delineation. MRI, with its superior soft tissue resolution, presents a promising alternative. However, MRI does not provide electron density information, which is essential for accurate dose calculation. This thesis develops advanced image processing methods to enhance MRI-guided adaptive radiotherapy, with a particular focus on generating synthetic CT (sCT) images from MRI. To increase model generalizability, the sCT synthesis is based on MRI/CT pairs collected from multiple clinical centers. An innovative non-rigid registration technique, using an intermediate sCT, has also been developed to improve the alignment between MRI and CT. This entire process is based on an unsupervised neural network. Furthermore, optimizing the MRI-linac workflow enables real-time treatment adaptation through daily MRI acquisitions. This allows for dynamic estimation and accumulation of delivered doses, providing support for clinical decision-making. Overall, this research contributes to enhancing the precision and reliability of MRI-guided adaptive radiotherapy, with the ultimate goal of improving clinical outcomes for patients.</dcterms:abstract>
