     <dcterms:abstract xml:lang="fr">La radiothérapie externe vise à délivrer de fortes doses de rayonnements ionisants en ciblant précisément les cellules tumorales, tout en épargnant les tissus sains. Cependant, les changements anatomiques qui se produisent au cours des semaines de traitement posent des défis importants à la précision de l'administration de la dose, soulignant la nécessité d'une radiothérapie adaptative (RTA). L'accumulation de dose basée sur le CBCT est apparue comme une méthode prometteuse pour le suivi et l'adaptation des traitements, mais elle se heurte à des incertitudes inhérentes à la qualité de l’image, au recalage multimodal et à la précision de la segmentation. Cette thèse présente une approche complète de l'évaluation de l'accumulation de dose en RTA utilisant le CBCT, en abordant explicitement les incertitudes liées à la génération de CT synthétique (sCT), à la segmentation et au recalage multimodale. En s’appuyant sur des méthodologies avancées d’apprentissage profond, nous proposons et évaluons des stratégies permettant une génération précise de sCT, des prédictions de segmentation sur les sCT, ainsi qu’un recalage multimodal fiable. L’évaluation et l’estimation des incertitudes à chaque étape du processus sont étudiées, dans le but d’améliorer la fiabilité des estimations de dose cumulée. La validation de ces méthodes a été réalisée en partie dans le cadre de défis internationaux, ce qui a permis d’obtenir des classements compétitifs et de démontrer leur viabilité clinique.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">External radiotherapy aims to deliver high doses of ionising radiation by precisely targeting tumour cells while sparing healthy tissue. However, anatomical changes occurring during the weeks-long treatment course present significant challenges to the accuracy of dose delivery, highlighting the need for adaptive radiotherapy (ART). CBCT-based dose accumulation has emerged as a promising method for monitoring and adapting treatments, but it faces uncertainties inherent in image quality, multimodal registration and segmentation accuracy. This thesis introduces a comprehensive approach to dose accumulation assessment in ART using CBCT, explicitly addressing the uncertainties inherent in synthetic CT (sCT) generation, segmentation and registration. Based on advanced deep learning methodologies, we propose and evaluate strategies for accurate sCT generation, robust segmentation predictions on sCTs and accurate multimodal registration. The evaluation and propagation of uncertainties through each step are studied, with the aim of improving the reliability of cumulative dose assessments. Validation of these methods has been achieved in part through international challenges, resulting in competitive rankings and demonstrating their clinical viability.</dcterms:abstract>
