L’intégration des tests moléculaires a transformé sa prise en charge, orientant les patients vers une médecine de précision où les traitements ciblés sont adaptés à leur profil tumoral et à leurs altérations génétiques spécifiques. Cependant, l’accessibilité à ces tests moléculaires reste soumise à des défis organisationnels, logistiques et financiers, engendrant des disparités sur le territoire français. Ce manuscrit fait un état des lieux du parcours du testing moléculaire, depuis son indication diagnostique jusqu’à son impact sur les stratégies thérapeutiques. Il met en évidence les enjeux actuels liés à la structuration des plateformes, au financement, aux délais de restitution des résultats et à l’uniformisation des pratiques. Des pistes d’amélioration sont évoquées, dans l’objectif d’assurer un accès équitable aux tests moléculaires et d’améliorer les chances de survie des patients atteints de CBNPC. Non-small cell lung cancer (NSCLC) accounts for the majority of lung cancers and remains a major public health issue. The integration of molecular testing has transformed its management, guiding patients toward precision medicine, where targeted treatments are tailored to their tumor profile and specific genetic alterations. However, access to these molecular tests remains subject to organizational, logistical, and financial challenges, leading to disparities across the French territory. This manuscript provides an overview of the molecular testing pathway, from its diagnostic indication to its impact on therapeutic strategies. It highlights current challenges related to the structuring of testing platforms, funding, turnaround times for results, and standardization of practices. Potential improvements are discussed with the aim of ensuring equitable access to molecular tests and enhancing the survival chances of patients with NSCLC.
