L’origine de la maladie reste incertaine bien que les symptômes soient décrits dès l’Antiquité. Le terrain génétique du diabète de type 1 ne pouvant expliquer à lui seul l’explosion de nouveaux cas, les scientifiques s’intéressent désormais aux influences environnementales. La dysbiose du microbiote intestinal observée chez les patients présentant une auto-immunité dirigée contre les cellules β de Langherans pourrait avoir un lien avec l’apparition de la maladie. L’observation de ces changements dans la flore intestinale et son possible lien avec la pathologie nous invite à réfléchir à un intérêt thérapeutique. Les injections pluri-quotidiennes d’insuline restent le traitement de référence du diabète de type 1, mais l’utilisation de méthodes permettant de restaurer la composition du microbiote intestinal pourrait représenter une avancée thérapeutique majeure dans la prise en charge de la pathologie. For several years, the number of cases of type 1 diabetes has continued to increase around the world. The origin of the disease remains uncertain although the symptoms have been described since Antiquity. The genetic background of type 1 diabetes cannot alone explain the explosion of new cases, scientists are now interested in environmental influences. The dysbiosis of the intestinal microbiota observed in patients with autoimmunity directed against Langherans β cells could be linked to the onset of the disease. The observation of these changes in the intestinal flora and its possible link with the pathology invites us to think about a therapeutic interest. Multiple daily insulin injections remain the standard treatment for type 1 diabetes, but the use of methods to restore the composition of the intestinal microbiota could represent a major therapeutic advance in the management of the pathology.
