Cette thérapie, abandonnée en Occident, connaît aujourd’hui un regain d’intérêt. Cependant, la phagothérapie peine à sortir du cadre expérimental. Ce travail s’interroge sur les raisons de ce blocage au stade des recherches. Cette synthèse de la littérature française et anglaise montre que malgré une forte présomption d’efficacité, le traitement n’a pas formellement montré son activité antibactérienne ni son intérêt thérapeutique dans les indications envisagées. De plus, son profil de tolérance et sa sécurité ne sont pas encore bien établis, que ce soit à l’échelle individuelle ou collective. Enfin, des produits de phagothérapie de qualité suffisante, bien standardisés, manquent – d’autant que leur développement continue d’être freiné par des problématiques réglementaires, économiques, ou encore organisationnelles. Phage therapy is an antibacterial treatment developed in the 1920s by Félix d'Hérelle, based on the administration of bacteria's natural viruses: bacteriophages. This therapy, which was abandoned in the West, is now regaining interest. However, phage therapy is struggling to move beyond the experimental stage. This Review explores the reasons for this stalemate at the research stage. This summary of the French and English literature shows that, despite a strong presumption of efficacy, the treatment has not formally demonstrated its antibacterial activity or its therapeutic value. Moreover, its safety and tolerability profiles are not yet well established, either on an individual or a collective scale. Finally, there is a lack of phage therapy products of sufficient quality and standardization - especially as their development continues to be hampered by regulatory, economic and organizational issues.
