     <dcterms:abstract xml:lang="fr">L’optimisation de nos stratégies de prise en charge des arythmies ventriculaires constitue un enjeu clinique majeur en cardiologie. Une meilleure compréhension des différents profils de sévérité chez les patients présentant des arythmies récidivantes, ainsi qu'une identification plus précise des profils répondant aux diverses options thérapeutiques, représentent des défis importants d’un point de vue clinique. Ces avancées potentielles sont représentées notamment par les approches permettant une meilleure compréhension des mécanismes soutenant certaines arythmies ventriculaires, couplées au développement de nouveaux outils de cartographie de ces anomalies. L’introduction récente d’une nouvelle source d’énergie ablative, l’électroporation, offre également des perspectives prometteuses, avec pour objectif la création de lésions plus profondes permettant d’éliminer certains substrats jusque-là difficilement accessibles avec la radiofréquence. Enfin, la radioablation stéréotaxique cardiaque, technique d’ablation non-invasive récente, offre déjà une nouvelle ligne thérapeutique efficace pour les patients en impasse thérapeutique, même si des défis précliniques, cliniques et technologiques importants attendent cette thérapie dans les années à venir.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">Optimizing the management of ventricular arrhythmias remains a major clinical challenge in cardiology. A critical step toward improving patient outcomes is achieving a better understanding of the varying severity profiles of patients with recurrent, clustered arrhythmias over time and identifying which of these profiles respond to different therapeutic approaches. Notable progress is emerging from approaches that enhance understanding of the underlying mechanisms involved in ventricular arrhythmias, alongside advances in mapping tools to better characterize these disorders. The recent introduction and development of a new ablative energy source, pulsed field ablation, offers promising potential by enabling the creation of deeper lesions to target arrhythmogenic substrates that are currently difficult to reach with radiofrequency ablation. Finally, stereotactic cardiac radioablation, a recent non-invasive ablation technique, provides a new therapeutic option and an effective alternative for patients in therapeutic impasse, although significant preclinical, clinical, and technological challenges remain on the path to fully implementing this new therapy.</dcterms:abstract>
