La biologie moléculaire a permis d’estimer sa prévalence jusqu’alors peu étudiée et sous-estimée du fait du manque de sensibilité de la microscopie. 41 507 prélèvements pour recherche d’IST ont été analysés au CHU de Rennes entre janvier 2020 et mars 2024 avec le panel Allplex STI Essential (Seegene), détectant T. vaginalis et 6 autres bactéries. Des qPCR spécifiques ciblant le gène de la béta-tubuline ou une région conservée répétée de T. vaginalis ont été réalisées pour étudier la spécificité du panel. Sur l’ensemble des tests effectués, 0.8% étaient positifs à T. vaginalis. La PCR simplex a confirmé la présence d’ADN dans 109/112 cas (97%). 44 patients étaient positifs à T. vaginalis seul et 82 étaient co-infectés avec au moins un germe pathogène détecté par le panel (Chlamydia trachomatis, Neisseria gonorrhoeae, Mycoplasma genitalium). 38% des patients étaient symptomatiques, parmi lesquels 54/128 avaient des symptômes typiques évocateurs de trichomonose. Plus de 60% ont reçu un traitement anti-infectieux, un imidazolé seul ou en association (secnidazole et/ou métronidazole). Nous avons ici décrit l’épidémiologie de 337 patients testés positifs pour T. vaginalis en plus de 3 ans. Le panel Allplex STI Essential est spécifique du parasite, mais certains échantillons mériteraient d’être séquencés afin de vérifier qu’il s’agit bien de T. vaginalis et non pas de T. tenax. Trichomonas vaginalis is an anaerobic flagellate protozoan responsible for a sexually transmitted infection (STI) which may be asymptomatic, under-diagnosed and therefore poorly treated. The Allplex STI Essential panel is specific for the parasite, but some samples would benefit from sequencing to verify that they are indeed T. vaginalis and not T. tenax.
