Matériel et méthodes : La cohorte comprend des patients atteints de cancer de prostate, prouvés histologiquement, initialement traités localement avec une intention curative, en récidive biochimique telle que définie par l’European Association of Urology. Les patients présentaient une récidive radiologique ganglionnaire pelvienne (jusqu'à la bifurcation aortique) avec un maximum de 5 lésions. La réirradiation a été définie selon le consensus ESTRO- EORTC, comme une irradiation présentant un chevauchement géométrique avec une irradiation antérieur. Résultats : Un total de 150 patients présentant des récidives sur 192 sites ganglionnaires a été inclus rétrospectivement dans 13 centres français. 80,7 % avaient reçu une prostatectomie radicale comme traitement primaire. Après un suivi médian de 30,5 mois, des toxicités gastro- intestinales (GI) de grade 2 et 3 et génito-urinaires (GU) sont survenues respectivement chez 1,9 %, 0 %, 7,1 % et 2,6 % des cas. Le taux de survie sans progression radiologique (rPFR) à 2 ans et à 3 ans était respectivement de 44,6 % (IC 95 % : 36,2 % - 55 %) et 32,9 % (IC 95 % : 24,8 % - 43,6 %). En analyse multivariée, une exposition antérieure à l'hormonothérapie (ADT), une ADT concomitante et le nadir du PSA après réirradiation étaient associés au rPFR. Le taux de survie sans hormonothérapie (ADT-FR) à 2 ans et à 3 ans était respectivement de 52,6%(IC95%:42%-65,9%)et35,1%(IC95%:24,7%49,8%). Enanalysemultivariée, l'exposition antérieure à l'ADT était associée à l'ADT-FR. Chez les patients présentant une récidive biochimique, une nouvelle PNMOR est survenue dans jusqu'à un tiers des cas. Conclusion : Cette étude montre un profil de toxicité rassurant de la réirradiation par stéréotaxie pour des PNMOR. Les résultats oncologiques à 3 ans sont encourageants. Aim: the CYGNUS study aims to assess both toxicity and efficacy of stereotactic body radiotherapy (SBRT) performed as salvage approach for pelvic nodal metachronous oligorecurrent (PNMOR) prostate cancer (PCa), in previously irradiated territory. Material and methods: Inclusion criteria were histologically history of proven PCa, locally treated with a radical intent and a biochemical relapse (BCR) as defined by the European Association of Urology guidelines. All patients had a radiological suspicion of PNMOR, defined as a recurrence occurring in pelvic nodal areas (up to the aortic bifurcation) with up to 5 suspect lymph nodes. Reirradiation was defined according to the ESTRO-EORTC consensus on re- irradiation as an irradiation with a geometrical overlap with a previous course of RT. Results: A total of 150 patients relapsing on 192 nodal sites were retrospectively included, from 13 French centers. The majority of patients (80.7%) received radical prostatectomy as primary treatment. After a median follow-up of 30.5 months, late grade 2 and grade 3 GI and GU toxicity occurred in respectively 1.9%, 0%, 7.1% and 2.6% of the cases. The 2-years and 3-years radiological progression-free rate (rPFR) reached respectively 44.6% (95% CI: 36.2% - 55%) and 32.9% (95% CI: 24.8% – 43.6%). In multivariate analysis, anterior exposition to ADT, concomitant ADT and PSA nadir following reirradiation was associated with rPFR. The 2-years and 3-years androgen deprivation therapy-free rate (ADT-FR) reached respectively 52.6% (95% CI: 42% - 65.9%) and 35.1% (95% CI: 24.7% – 49.8%). In multivariate analysis, anterior exposition to ADT was associated with ADT-FR. In patients experiencing biochemical relapse, further PNMOR occurred in up to a third of the cases. Conclusion: The CYGNUS retrospective study supports the safety of reirradiation with SBRT for PNMOR. Oncological outcomes are encouraging. Long-term results from the ReCare prospective cohort are eagerly awaited to provide high-quality data on reirradiation with SBRT.
