Les obèses peuvent être victimes de grossophobie médicale: une stigmatisation effective ou perçue lors des soins, qui a des effets néfastes sur le poids. Le but de cette étude est d’explorer les expériences médicales et le ressenti des patients face à la question du poids pour optimiser leur prise en charge. Matériels et méthodes: Etude qualitative à l’aide d’entretiens semi dirigés interrogeant quinze adultes en situation d’obésité, actuellement ou par le passé. Résultats: Les patients atteints d’obésité sont en souffrance physique et psychique. Même si d'apparence ils minimisent leur état de santé ce n’est pas un sujet tabou pour eux: ils acceptent de se peser et sont conscients de l'intérêt d’une prise en charge médicale. La grossophobie est visible chez certains médecins qui adoptent des attitudes moralisatrices et culpabilisantes sans solution probante. Les patients attendent un accompagnement bienveillant et personnalisé mais n’ont pas de réponse universelle concernant l’approche à adopter. Conclusion: Les patients ont besoin d’un accompagnement bienveillant ; repérer leurs souffrances et s'intéresser à leur histoire contribue au développement d’une relation de confiance adaptée pour aborder la question du poids en médecine générale. Objectives: Obesity is a complex pathology, associated with co-morbidities, which affects 17% of the French population. Obese people may be victims of medical grossophobia: actual or perceived stigmatisation during care, which has harmful effects on weight. The aim of this study was to explore patients' medical experiences and feelings regarding weight in order to optimise their care. Materials and methods: Qualitative study using semi-directed interviews questioning fifteen adults with obesity, currently or in the past. Results: Patients suffering from obesity are in physical and psychological distress. Even if they appear to minimise their condition, it is not a taboo subject for them: they agree to weigh themselves and are aware of the benefits of medical treatment. Grossophobia is visible among some doctors, who adopt moralising and guilt-inducing attitudes without any convincing solutions. Patients expect caring, personalised support but have no universal answer as to the approach to adopt. Conclusion: Patients need caring support; identifying their suffering and taking an interest in their history contributes to the development of a relationship of trust that is suited to tackling the issue of weight in general practice.
