Son évolution vers la réalité virtuelle (VR) offre un outil à la frontière entre réalités théorique et pratique et, surtout, inlassablement reproductible. Mais bien qu'il y ait de plus en plus d'articles scientifiques parlant de l'intérêt de la simulation VR en chirurgie arthroscopique, peu rapportent sa validité lors de la transposition des performances du simulateur vers le patient réel. Nous avons pu conduire différentes études dans un cadre structuré couplant de façon innovante formation et recherche en pédagogie afin d’évaluer la pertinence de ces simulateurs dans la formation initiale du chirurgien orthopédiste. L’objectif de la première étude fut de caractériser l’expertise chirurgicale sur simulateur VR d’arthroscopie à travers une analyse approfondie et innovante des trajectoires instrumentales. L’objectif de la seconde étude fut de confirmer l’acquisition et la transposition des compétences arthroscopiques de base du simulateur VR sur sujet anatomique d’un groupe d’internes en phase socle issus de notre réseau inter-régional dans le cadre d’une exploration arthroscopique diagnostique et méniscectomie partielle médiale guidée du genou. L’objectif de la troisième étude fut d’analyser le gain en compétences non techniques grace à cet apprentissage structuré notamment sur la gestion du stress en analysant la variabilité de fréquence cardiaque. L’un de nos objectifs futurs est de pouvoir transposer ces programmes structurés d’entraînement à d’autres apprentissages procéduraux notamment en réalité virtuelle immersive. Simulation-based arthroscopy training is becoming an established part of national orthopaedic surgery procedural training programs. Its evolution towards virtual reality (VR) offers a tool at the frontier between theoretical and practical realities and, above all, tirelessly reproducible. But although there are a growing number of scientific articles talking about the value of VR simulation in arthroscopic surgery, few report its validity when transposing simulator performance to the real patient. We have been able to conduct a number of studies within a structured framework that combines training and pedagogical research in an innovative way, in order to assess the relevance of these simulators in the initial training of orthopaedic surgeons. The aim of the first study was to characterize surgical expertise on a VR arthroscopy simulator, using an in-depth innovative analysis of instrumental trajectories. The aim of the second study was to confirm the acquisition and transposition of the VR simulator's basic arthroscopic skills to an anatomical subject by a group of base-phase interns from our inter-regional network, in the context of diagnostic arthroscopic exploration and guided partial medial meniscectomy of the knee. The aim of the third study was to analyze the gain in non-technical skills thanks to this structured training, in particular stress management, by analyzing heart rate variability. One of our future objectives is to transpose these structured training programs to other procedural learning, notably in immersive virtual reality.
