     <dcterms:abstract xml:lang="fr">La chirurgie reconstructrice vise à corriger des pertes de substance accidentelles ou secondaires à des traitements, en utilisant des techniques parfois avancées comme la microchirurgie et les allo-transplantations de tissus composites (ATC). Malgré les progrès, ces techniques présentent d’importantes limites, réduisant leur utilisation en dernier recours, et menant parfois à des situations d’impasse thérapeutique. La machine de perfusion, une innovation issue de la transplantation d'organes solides, pourrait améliorer ces pratiques en reproduisant la circulation sanguine pour préserver les greffons et optimiser les résultats, tant en transplantation (ATC) qu’en chirurgie reconstructrice autologue. Cette thèse explore son application dans trois domaines de la Chirurgie plastique et reconstructrice : la préservation prolongée des greffons en ATC, l'optimisation de la reconstruction autologue de dernier recours par lambeaux fasciocutanés libres sans anastomoses microchirurgicales, et l'ingénierie tissulaire afin de recréer des organes et tissus à partir de matrices acellulaires, sans risque de rejet.</dcterms:abstract>
     <dcterms:abstract xml:lang="en">Reconstructive surgery aims to correct accidental or treatment-related tissue defects using advanced techniques such as microsurgery and Vascularized Composite allotransplantation (VTA). Despite recent advancements, these techniques present significant limitations, restricting their use to last-resort situations and sometimes leading to therapeutic dead ends. Machine perfusion techniques, an innovation derived from solid organ transplantation, could improve these practices by mimicking blood circulation to preserve grafts and optimize outcomes in both VCA and autologous reconstructive surgery. This thesis explores its application in three areas in plastic and reconstructive surgery: the prolonged preservation of VCA grafts, the optimization of last-resort autologous reconstruction using free fasciocutaneous flaps without microsurgical anastomoses, and tissue engineering approaches to recreate organs and tissues from acellular matrices with no risk of rejection. </dcterms:abstract>
