A ce jour, les facteurs impliqués dans l’équilibre notamment pour des patients atteints d’un AVC droit restent sujets à discussion avec notamment une probable implication des troubles de la cognition spatiale. L’objectif de cette étude était de déterminer si les troubles de la cognition spatiale expliquent en partie la précarité de la fonction d'équilibre au moment où le patient acquiert la position debout après un AVC droit. Elle visait également à identifier les marqueurs des troubles de la cognition spatiale qui interfèrent avec la fonction d'équilibre. Méthodes : Une étude transversale d'un essai randomisé multicentrique incluant des patients avec un AVC droit capables de tenir debout et comprenant une évaluation initiale de leur motricité, spasticité, sensibilité, latéropulsion (SCP) et des évaluations de leur cognition spatiale (test d’héminégligence, le droit devant et l’axe longitudinal). L’équilibre était évalué à l’aide du Berg Balance Scale (BBS). Un seuil de moins de 45/56 a été défini pour indiquer une perturbation sévère de l’équilibre. Les facteurs au temps initial ont été analysés par des régressions logistiques univariée et multivariée. Résultats : 82 patients ont été analysés (moyenne d’âge (années) : 59.3 ± 10.3), délai post-AVC (jours) : 91.3 ± 59.6) dont 61 patients avec un score d’équilibre précaire (BBS < 45/56). L’analyse multivariée révèle que plus le score de motricité au membre inférieur parétique est haut, plus ce facteur est protecteur (OR 0,89 ; IC95% [0,81 ; 0,96] ; p = 0,0027). En revanche, la présence d’une latéropulsion (OR 6 ; IC95% [1,14 ; 31,67] ; p = 0,0349) et d’une négligence (OR 19,88 ; IC95% [1,59 ; 248,8] ; p = 0,0204) sont associés au risque de présenter un BBS < 45/56. Discussion et conclusion : La présence d’un déficit de motricité au membre inférieur, d’une héminégligence et d’une latéropulsion sont les 3 facteurs indépendants reliés à la précarité de l’équilibre dans les suites d’un AVC hémisphérique droit. Ces trois facteurs peuvent raisonnablement être ciblés dans la rééducation des patients AVC droits, capable de tenir debout et présentant la persistance d’une atteinte de la fonction d’équilibre. Introduction: One of the consequences of a stroke is impaired balance, which increases the risk of falls and loss of independence. To date, the factors involved in balance, particularly in patients with right stroke, remain open to debate, with spatial cognition disorders probably being involved. The aim of this study was to determine whether spatial cognition disorders partly explain the precariousness of the balance function at the time when the patient acquires the upright position after a right stroke. It also aimed to identify markers of impaired spatial cognition that interfere with balance function. Methods: A cross-sectional study of a multicentre randomised trial including patients with right stroke able to stand and including an initial assessment of their motor skills, spasticity, sensitivity, lateropulsion (SCP) and assessments of their spatial cognition (hemineglect test, the straight ahead and longitudinal axis). Balance was assessed using the Berg Balance Scale (BBS). A threshold of less than 45/56 was defined to indicate a severe disturbance of balance. Factors at baseline were analysed by univariate and multivariate logistic regressions. Results: 82 patients were analysed (mean age (years): 59.3 ± 10.3), post-stroke time (days): 91.3 ± 59.6) including 61 patients with a precarious balance score (BBS < 45/56). Multivariate analysis revealed that the higher the motor score for the paretic lower limb, the more protective this factor was (OR 0.89; 95% CI [0.81; 0.96]; p = 0.0027). On the other hand, the presence of lateropulsion (OR 6; 95% CI [1.14; 31.67]; p = 0.0349) and neglect (OR 19.88; 95% CI [1.59; 248.8]; p = 0.0204) were associated with the risk of presenting a BBS < 45/56. Discussion and conclusion: The presence of a motor deficit in the lower limb, hemineglect and lateropulsion are the 3 independent factors related to the precariousness of balance in the aftermath of a right hemispheric stroke. These three factors can reasonably be targeted in the rehabilitation of right stroke patients who are able to stand and have persistent impairment of balance function.
