Appliquée à la médecine, la métaphore permettrait au patient de pouvoir ré-explorer sa propre histoire à la recherche d’un nouvel éclairage pouvant déboucher sur un changement cognitif, émotionnel ou comportemental. Son utilisation consciente par les médecins généralistes est encore à décrire. L’objectif de cette étude est de recueillir l’expérience de terrain des médecins généralistes quant à leurs usages des métaphores en pratique et leurs perceptions de la métaphore comme outil de consultation. Méthode : Étude qualitative par entretien semi-dirigé individuel auprès de 8 médecins généralistes du bassin rennais. Le recrutement a été réalisé entre Octobre 2023 et Février 2024 par échantillonnage intentionnel jusqu’à suffisance des données. Une analyse ouverte axiale et intégrative du verbatim a été effectuée, inspirée de la théorisation ancrée, permettant une modélisation d’un modèle explicatif. Résultats : Notre étude montre un usage large des métaphores par les médecins généralistes. C’est un vecteur d’information et d’émotion qui participe à créer un espace réflexif et relationnel commun au médecin et au patient. La métaphore, pensée dans des utilisations thérapeutiques, permet de préciser des sensations, de rendre accessible et utilisable l’information et le raisonnement qui s’y rapportent, d’accéder aux représentations, de sortir d’une situation complexe, de penser et renforcer la relation médecin patient, de favoriser l’autonomie en facilitant la mémorisation de l’information, d’accompagner une modification comportementale. Les médecins généralistes héritent d’un catalogue de métaphores, issu de leur formation : il y a celles auxquelles ils ont été exposés et celles qu’ils ont créées pour leur propre compte lorsque leur raisonnement était d’inspiration métaphorique. Ils utilisent également celles issues de leurs vécus et de leur expérience physique du monde. Des limites et des précautions à son utilisation sont mises en avant pour que la métaphore reste bénéfique au patient. Conclusion : Les métaphores sont omniprésentes pour les médecins. Bien sûr, le langage imagé ne remplace ni l’expérience accumulée, ni la formation technique, ni l’écoute du patient. Il n’en reste pas moins un moyen par lequel le médecin peut faire passer un message. La métaphore, en offrant au patient à réfléchir et à partager, favorise son adhésion, sa motivation et son autonomisation. La métaphore devra être adaptée au contexte et aux besoins, en faisant attention aux représentations qu’elles convoquent. Alors, si elle est utilisée en ayant conscience de ses possibilités et de ses précautions d’usage, elle aura des vertus thérapeutiques : elle sera cognitive, relationnelle et centrée-patient. Introduction : Metaphor is a cognitive process permitting to understand a subject (and experience it) through comprehension (and experience) of another subject. Applied to medicine, metaphor could allow a patient to re-explore his own story through a new insight, leading to a cognitive, emotional or behavioural change. Its conscious use by general practitioners is yet to be described. The goal of this study is to collect field-experience of general practitioners in regard to the practical usage of metaphors in practice, and the perception of metaphor as a consultation tool. Method : Qualitative study by semi-structured individual interviews with eight general practitioners based in Rennes basin, France. Recruitment has been realized from October 2023 to February 2024 by intentional sampling until data sufficiency. An open axial and integrative analysis of the verbatim was carried out, allowing modelling of data according to grounded theory method. Results : This study demonstrates a large use of metaphors by general practitioners. It is an informational and emotional vector that participates in creating a reflexive and relational space, common to the doctor and the patient. Metaphors, as in therapeutical uses, allow to specify sensations, make accessible and usable information and reflexion that relate to it, permit access to representations, getting out of a complex situation, think and strengthen the doctor-patient relationship, favour autonomy by facilitating memorization of information, accompany behavioural modification. The general practitioners inherit a metaphor catalogue from their training : those they have been exposed to, and those they create for their own, if they had metaphorical thinking. They also use those from their own livings and physical experiences of the world. Limits and precautions to their use are put forward so that the metaphor remains beneficial for the patient. Conclusion: Metaphors are omnipresent for doctors. Of course, representative language does not replace nor cumulative experience, nor technical formation, nor patient listening. Yet, it remains a way to share a message for general practitioners. Metaphor, by offering reflexion and sharing to the patient, favours adhesion, motivation and empowerment. Metaphor should be adapted to contexts and needs, by taking care of representation that it conveys. Then, if used being aware of its possibilities and precautions, it will have therapeutic virtues : it will be cognitive, relational and patient-centred.
