La sarcoïdose est une maladie multisystémique qui atteint les poumons dans plus de 90% des cas. Dans 25%, elle évolue vers une forme chronique fibrosante qui contribue fortement à la morbi-mortalité de la maladie. La répétition des scanners thoraciques durant le suivi de ces patients est à l'origine d'une élévation du risque de cancers radio-induits. Les progrès récents du scanner et des techniques de reconstructions itératives permettent la réalisation de scanners peu irradiants. Objectif. Evaluer la concordance diagnostique du scanner ultra-low-dose (ULD) et du scanner normodosé pour l'analyse de l'atteinte fibrosante dans la sarcoïdose thoracique. Matériel et méthodes. Étude prospective et multicentrique menée sur 171 patients atteints d'une sarcoïdose pulmonaire, entre mai 2017 et octobre 2021. Réalisation successive d'un scanner normodosé et d'un scanner ULD pour chaque patient, puis lecture en aveugle par trois radiologues. Calculs du coefficient de corrélation intra-classe (CCI) et du coefficient Kappa de Cohen. Résultats. La concordance diagnostique entre l'acquisition normodosée et l'acquisition ULD pour quantifier la fibrose pulmonaire était excellente avec un CCI de 0,919 (IC à 95% : 0,891-0939). La reproductibilité inter-observateur n'était pas inférieure avec l'acquisition ULD en comparaison à l'acquisition normodosée. Conclusion. Cette étude a montré une excellente concordance diagnostique entre les acquisitions ULDs et standards pour la quantification visuelle de l'atteinte fibrosante. Ces résultats suggèrent que le scanner ULD pourrait être utilisé dans le suivi des patients atteints de sarcoïdose thoracique. Introduction : In around 20% of cases of sarcoidosis, pulmonary granulomas progress to severe fibrosing lesions, which contribute greatly to the morbidity and mortality of the disease. Sarcoidosis most oben requires repeated chest computed tomography (CT) scans, resulting in a substantial accumulation of radia;on exposure and increasing, at least theoretically, the risk of radiation-induced cancer. Ultra-low dose (ULD) CT is an alternative to standard CT, enabling radiation levels to be drastically reduced. The aim of this study was to assess the concordance of ULD CT and standard CT in chest sarcoidosis. Material-methods : In this multicenter prospective study, 171 patients with proven thoracic sarcoidosis were included. Standard and ULD CT were carried out consecutively and reviewed by three radiologists. Concordance between standard and ULD CT was assessed by the calculation of the intra-class correlation coefficient, the Cohen’s kappa coefficient and the graphical methods of Bland and Altman. Results : Agreement between standard and ULD CT to quantify pulmonary fibrosis was very good with an ICC of 0.919 (95%CI: 0.891-0.939). The mean difference in fibrosis scoring between standard and ULD CT was 4.46%. Agreement between standard and ULD CT to assess pulmonary nodules, consolidation and the Drent score was good with ICC ranging from 0.806 to 0.875. For most CT findings, interobserver agreement was slightly better in ULD than in standard CT. Conclusion : Overall, this study showed a very good concordance between standard and ULD CT for visual quantifica;on of pulmonary fibrosis. These findings suggest that ULD CT could be safely used in the follow-up of patients with pulmonary sarcoidosis.
