La proportion de patients « long survivants » est estimée entre 8 et 16% selon les études. Cette population de patients, dont il n’existe pas de définition consensuelle, reste mal connue. Matériel et méthodes : L'étude BREATH est une étude observationnelle rétrospective monocentrique incluant tous les patients porteurs d'un CBNPC avancé traités par immunothérapie et présentant une maladie contrôlée pendant au moins 12 mois après la première injection d'immunothérapie. L’objectif principal est la description des caractéristiques de cette population. Les données ont été recueillies d’après les dossiers médicaux. Résultats : L'étude a inclus 49 patients. Après 51 mois de suivi médian, la médiane de SSP ou de SG n’est pas atteinte. A 36 mois, la SSP était de 64,7% et la SG de 91,6%. Les patients ayant reçu un schéma complet d'immunothérapie (2 ans) ont eu une meilleure SSP (HR 0,046 ; IC à 95 % [0,14-0,98] ; p = 0,03) que ceux ayant reçu un schéma incomplet (< 2 ans). Le taux d'albumine ≥ 35 g/L à l'initiation de l'immunothérapie était le seul facteur associé à une SSP prolongée en analyse multivariée (HR 0,2 ; IC à 95 % [0,04-0,89] ; p = 0,035). La présence de maladie métabolique résiduelle à la fin du schéma d'immunothérapie ou une éventuelle radiothérapie complémentaire sur les résidus hypermétaboliques n'a pas d'impact significatif sur la SSP. Conclusion : Les patients porteurs d’un CBNPC avancé et présentant un contrôle prolongé sous immunothérapie représentent 12.3 % des patients de notre cohorte de vie réelle. La survie de ces patients est exceptionnellement longue et témoigne d’un changement de paradigme dans nos pratiques. La conduite d’un schéma d’immunothérapie complet (2 ans) semble associée à une meilleure SSP. Introduction: Immune checkpoint inhibitors have improved the prognostic of non-small-cell lung cancer (NSCLC). Depending on studies, the proportion of long survivors is estimated between 8 to 16 %. Characteristics of those patients is not well known. Method: BREATH study is a monocentric retrospective observation of patients with advanced NSCLC, treated by immune checkpoint inhibitors, and having a controlled disease during at least 12 months after the first injection. The main outcome is to describe patients’ characteristics. Data used are coming from medical records. Results: The study included 49 patients. After 51 months of median follow-up, the median progression-free survival (PFS) or median overall survival (OS) is not reached. After 36 months, the PFS was 64.7% and OS was 91.6%. Patients treated with a complete immunotherapy protocol (2 years) have a better PFS (HR 0,046; IC à 95 % [0,14-0,98] ; p = 0,03) than the ones treated with an incomplete protocol (less than 2 years). The albumin rate ≥ 35 g/L at the beginning of immunotherapy was the only factor linked to long PFS in multivariate analysis (HR 0,2; IC à 95 % [0,04-0,89]; p = 0,035). Presence of a residual metabolic disease at the end of immunotherapy, or an optional additional radiotherapy on hypermetabolic residues, does not have significative impact on PFS. Conclusion: Patients having advanced NSCLC long-term controlled by immune checkpoint inhibitors represent 12.3% of our real-life cohort. Survival of those patients is exceptionally long and testifies a paradigm shift in our practices. Only a complete immunotherapy protocol (2 years) seems to be linked to better PFS.
