Au cours du bilan préopératoire de l’endocardite infectieuse (EI), la réalisation d’une évaluation coronarienne peut etre suggérée dans certains cas. Vu le faible niveau de preuve de cette recommandation, nous avons mené une étude afin de mieux appréhender le rapport bénéfices/risques de cet examen. Matériels et méthodes. Etude observationnelle rétrospective multicentrique, menée dans trois CHU français. Le critère de jugement principal était la prévalence de la réalisation de l’évaluation coronarienne. Résultats. Trois cent vingt-trois patients ont été inclus : 184 (57%) ont eu une évaluation coronarienne (groupe C) – 149 par coronarographie, 35 par coroscanner – tandis que les 139 autres (43%) n’ont pas eu cette évaluation (groupe NC). L’évaluation coronarienne a permis de dépister 37 patients avec une coronaropathie significative, parmi lesquels 13 ont eu un pontage aorto-coronarien per chirurgical. Conclusion. Les recommandations actuelles sont difficilement applicables : l’évaluation coronarienne ayant été conduite chez seulement 57% de notre cohorte, alors que 98% en avait l’indication. La coronarographie expose à très peu de risques et permet de dépister une coronaropathie significative chez un malade sur cinq, ceci pouvant aboutir ensuite à un geste de revascularisation coronarienne en peropératoire. De futures études sont nécessaires afin d’identifier les facteurs pronostiques de la réalisation d’une revascularisation. Introduction: During the preoperative assessment of infective endocarditis (IE), the need for coronary evaluation may be suggested in certain cases. Given the limited level of evidence for this recommendation, we conducted a study to better understand the benefits and risks of this examination. Materials and Methods: This was a retrospective multicenter observational study conducted in three French university hospitals. The primary outcome was the prevalence of coronary evaluation. Results: A total of 323 patients were included, with 184 (57%) undergoing coronary evaluation (C group) – 149 via coronary angiography and 35 via coronary CT angiography – while the remaining 139 (43%) did not undergo this evaluation (NC group). Coronary evaluation detected significant coronary artery disease in 37 patients, with 13 of them subsequently undergoing aortocoronary bypass surgery. Conclusion: Current recommendations are challenging to implement, as coronary evaluation was performed in only 57% of our cohort, despite 98% meeting the criteria for it. Coronary angiography carries minimal risks and can identify significant coronary artery disease in one out of five patients, potentially leading to coronary revascularization during surgery. Future studies are needed to identify prognostic factors for undergoing revascularization.
