Actuellement, les traitements proposés dans les recommandations sont restreints. Les inhibiteurs du SGLT2 ont récemment démontré une efficacité majeure dans l'HFpEF. Nous avons donc voulu explorer l'impact de ces traitements (dapagliflozine ou empagliflozine) sur la fonction myocardique et la capacité fonctionnelle. Méthodes : Pour cette étude, nous avons recruté 46 patients atteints de CMH non obstructive d'août 2019 à janvier 2023 ; 18 étaient traités par dapagliflozine ou empagliflozine et 28 témoins ont été inclus. Les données de l'échocardiographie d'effort et de l’épreuve d’effort cardiorespiratoire ont été recueillies avant et après l'introduction du traitement. Nous avons comparé les pics de VO2 moyens avant et après l'introduction des gliflozines. L’objectif principal était d'identifier les paramètres prédictifs d'une amélioration du pic de VO2. Résultats : Quarante-six patients symptomatiques atteints de CMH ont été inclus. Soixante-dix pourcents étaient des hommes, l'âge moyen était de 58,1±12,2 ans, le pic de VO2 moyen était de 21,6±7 ml/kg/min à l’inclusion. Le strain de la paroi libre du ventricule droit est un facteur prédictif significatif d’une amélioration du pic de VO2 chez les patients traités avec des inhibiteurs du SGLT2 (R= 0,76 ; p<0,05). Le strain de la paroi libre du ventricule droit est le seul paramètre échographique présentant une interaction avec le traitement (p d’interaction = 0,05). Le pic de VO2 et les autres paramètres de l’épreuve d’effort cardiorespiratoire n’ont pas significativement variés après 5 mois de traitement. Conclusion : Les inhibiteurs du SGLT2 n’ont pas significativement améliorés la capacité fonctionnelle de tous les patients CMH. Néanmoins, avoir un strain de la paroi libre du VD normal est un facteur prédictif significatif en faveur d’une amélioration du pic de VO2. Background: Patients with a diagnosis of non-obstructive hypertrophic sarcomeric cardiomyopathy (HCM) often present with symptoms very close to heart failure with preserved ejection fraction (HFpEF). So far, treatments proposed in guidelines remain limited. SGLT2 inhibitors recently demonstrated a substantial efficacy in HFpEF. Hence, we sought to explore the impact of SGLT2 inhibitors (dapagliflozin or empagliflozin) on myocardial function and their consequences on functional capacity. Methods: For this study, we recruited 46 non-obstructive HCM patients from August 2019 to January 2023; 18 were treated with dapagliflozin or empagliflozin and 28 controls were enrolled. Exercise echocardiography and cardiopulmonary exercise testing data were performed before and after SGLT2 inhibitor’s introduction. We looked at mean changes in peak VO2 after gliflozins introduction. The primary endpoint was to identify parameters predicting an improvement of peak VO2. Results: We enrolled and followed 46 symptomatic HCM patients. Seventy percent were male, mean age was 58.1±12.2 years, baseline peak VO2 was 21.6±7 ml/kg/min. RV free wall strain was a strong significant predictor of peak VO2 improvement in patients treated with SGLT2 inhibitors (R= 0.762; p<0.05). RV free wall strain is the only echocardiographic parameters showing interaction with the treatment (p for interaction = 0.052). Overall, peak VO2 and other CPET parameters did not significantly change after 5-months’ treatment. Conclusion: SGLT2 inhibitors did not impact on functional capacity in all HCM patients. Nevertheless, preserved RV free wall strain was a strong and significant predictor of peak VO2 improvement.
