Cette étude avant-après se propose d’évaluer l’impact d’une meilleure communication entre Ville et Hôpital sur les taux de réhospitalisation des sujets de plus de 75 ans. Matériel et méthodes : L’étude consistait à contacter le médecin traitant avant la sortie du patient afin de la préparer et à fournir une lettre de liaison à la sortie. L’inclusion a eu lieu du 1er mars au 31 août 2020 et 2021, pour les patients de plus de 75 ans admis en médecine polyvalente et rentrant à domicile. Résultats : Les réhospitalisations ont diminué sans significativité statistique, y compris selon le score de risque TRST, sur des cohortes comparables. Le profil des patients ré-hospitalisés à 7 et 30 jours semble différent. Conclusion : La complexité des causes de réhospitalisations nécessite la convergence d’actions ciblées, en plus d’un meilleur lien Ville-Hôpital. Introduction: The lack of the link between primary care and Hospital is a public health issue in France, a source of early and avoidable re-hospitalizations and the morbidity and mortality they generate. This before-after study aims to assess the impact of better communication between primary care and Hospital on the re-hospitalization rates of subjects over 75 years of age. Material and methods: The study consisted of contacting the general practitioner before the patient's discharge in order to prepare it and providing a liaison letter at discharge. Inclusion took place from March 1 to August 31, 2020 and 2021, for patients over the age of 75 hospitalized to medicine’s units and returning home. Results: Rehospitalizations decreased without statistical significance, including according to the TRST risk score, on comparable cohorts. The profile of patients re-hospitalized at 7 and 30 days seems different. Conclusion: The complexity of the causes of rehospitalizations requires the convergence of targeted actions, in addition to a better communication between primary care and Hospital.
