Le traitement des événements thrombotiques repose sur un traitement anticoagulant au long court par AVK. Les anticoagulants oraux directs (AOD) ne sont pas recommandés en raison d’un sur risque de récidive thrombotique observé dans plusieurs essais randomisés. L’objectif de l’étude était donc d’identifier dans la cohorte rennaise de patients avec un SAPL thrombotique les facteurs associés à une récidive thrombotique et de décrire le phénotype des patients en fonction du traitement anticoagulant choisi. Nous avons recueilli les données de 207 patients atteints d’un SAPL thrombotique sur une cohorte initiale de 655 patients identifiés par un logiciel de screening automatisé. Cette analyse rétrospective a permis de mettre en évidence des facteurs de récidives thrombotiques déjà connus comme l’absence de traitement antithrombotique après l’évènement initial (HR = 3.43 [1.06 ; 11.08]), un traitement par antiagrégant plaquettaire seul après l’évènement initial (HR = 4.17 [1.53 ; 11.36]) et d’autres facteurs comme la présence d’un lupus systémique (HR = 4.27 [1.31 ; 13.92]) ou d’un SAPL obstétrical avant le 1er évènement thrombotique (HR = 7.70 [2.70 ; 21.96]). Il n’y avait pas de différence sur le risque de récidive thrombotique entre les patients traités par AVK (n=123) et les patients traités par AODs (n=50). La prescription d’AVK par rapport à un AOD était associée à la présence d’une triple positivité des anticorps antiphospholipides (OR = 7.90 [1.35 ; 46.21]) et l’absence d’un évènement thrombotique veineux initial (OR = 0.01 [0.00 ; 0.25]). Cette cohorte confirme la place des AODs dans le traitement du SAPL thrombotique pour les patients ayant présenté un évènement initial veineux et avec un profil sérologique simple positif. Antiphospholipid syndrome (APS) is an autoimmune disease characterized by the presence of autoantibodies (anti-cardiolipin, anti-β2GP1, and lupus anticoagulant) associated with arterial, venous, or microcirculatory thrombotic events and/or obstetrical events (miscarriages, fetal death, and preterm deliveries). The treatment of thrombotic events is based on long-term anticoagulant therapy with vitamin K antagonist (VKA). Direct oral anticoagulants (DOACs) are not recommended as first-line therapy because of an increased risk of recurrent thrombosis observed in several randomized trials. The objective of the study was to identify in the Rennes cohort of thrombotic SAPL patients the clinical and biological factors associated with recurrent thrombosis and describe the patients phenotype according to the anticoagulant treatment. We collected data from 207 patients with thrombotic APS from an initial cohort of 655 patients identified by automated screening software. This retrospective analysis allowed us to highlight factors for thrombotic recurrence that were already known, such as the absence of antithrombotic treatment after the initial event (HR = 3.43 [1.06; 11.08], treatment with antiplatelet treatment alone after the initial event (HR = 4. 17 [1.53; 11.36]), but also other factors such as the presence of systemic lupus erythematosus (HR = 4.27 [1.31; 13.92]) or obstetrical APS before the primary thrombotic event ( HR = 7.70 [2.70; 21.96]). We found no difference between patients treated with VKA (n=123) or DOACs (n=50) on the risk of recurrent thrombosis. Prescription of VKA versus DOACs was associated with the presence of triple APL positivity (OR = 7.90 [1.35; 46.21]) and the absence of an initial venous thrombotic event (OR = 0.01 [0.00; 0.25]). This cohort confirms the use of DOACs in the treatment of thrombotic APS for patients with a primary VTE event and a single APL positivity profile.
