Indications et résultats Métroplastie d’agrandissementinfertilitémalformation utérineutérus en Tendomètre fin Stérilité Utérus Chirurgie Utérus Malformations Chirurgie gynécologique Endomètre‎ Objectif : Evaluer dans une population de patientes nullipares et infertiles, avec un facteur utérin d’infertillité identifié, qu’il s’agisse d’une dysmorphie à type d’utérus en T ou encore d’un endomètre fin, le résultat sur la fertilité après réalisation d’une métroplastie d’agrandissement. Le critère de jugement principal porte sur la survenue d’une naissance vivante à 18 mois post chirurgie. Design: Il s’agit d’une étude retrospective observationnelle monocentrique réalisée entre entre le 1er janvier 2015 et le 31 décembre 2019 à la clinique Mutualiste de la Sagesse. Resultats : 43 patientes ont été inclues dans l’étude. En post chirurgie le taux de naissance vivante était de 53.5% dont 27.9 % à 18 mois post chirurgie, contre 0% avant la chirurgie. Nous n’avons relevé aucune complication per ou post opératoire. Conclusion : La métroplastie d’agrandissement semble améliorer significativement les chances de naissance vivante dans une population de patientes nullipares et infertiles souffrant a minima d’une pathologie utérine à type d’utérus en T ou d’endomètre fin. Objective: To evaluate hysteroscopic metroplasty in the management of nulliparous and infertile patients with T-shaped uterus type dysmorphia, thin endometrium or a poor distensibility cavity. The primary endpoint was the occurrence of a live birth at 18 months post-surgery. Study design: This is a single-center retrospective observational study conducted between 1er January 2015 and 31 December 2019. Results : Fourty three nulliparous patients were included with an average of 5.2+/-2.4 years of primary infertility, including 84.2% patients who had at least one IVF cycle prior to the surgery. The post-surgery live birth rate was 27.9% at 18 months post-surgery and 53.5% during the total post-operative follow-up (mean of follow-up 4.5 years +/- 1 year). Pregnancies were obtained spontaneously after surgery for 8/28 (28.6%) of the patients who were undergoing medically assisted reproduction before surgery. There were no intra- or postoperative complications recorded. Conclusion: Hysteroscopic metroplasty appears to significantly improve the chances of live birth in a population of nulliparous and infertile patients with at least one uterine pathology such as T-shaped uterus or thin endometrium. Furthermore, there is also a place for spontaneous pregnancy attempts post hysteroscopic metroplasty.
