Obstruction des artères Artériopathies Thérapeutique Membre inférieur Lipides Chirurgie vasculaire Objectif : Décrire, chez les patients atteints d’une artériopathie oblitérante des membres inférieurs (AOMI), la prévalence des patients bénéficiant d'un traitement médical optimal et respectant les objectifs lipidiques selon les recommandations internationales et comparer ces données entre les patients ayant des antécédents de chirurgie vasculaire des membres inférieurs et ceux n'en ayant pas. Méthode : Étude rétrospective monocentrique incluant des patients présentant une AOMI symptomatique. Les données ont été recueillies de janvier 2020 à mai 2022 chez les patients ayant consulté pour un test de marche Résultats : 225 patients ont été inclus : 51.1% des patients ont reçu une trithérapie, 12.4% des patients ont un LDLc dans la cible <0.55g/l et 27.6% des patients atteignent la cible <0.70 g/l. Concernant les seuils de triglycérides, 64.4% sont dans les seuils cibles <1,50g/l. Chez les patients opérés des membres inférieurs, 52.9% ont bénéficié d'un traitement médical optimal contre 50,0% chez les patients non opérés (p=0.6687), 10.6% sont dans la cible de LDLc <0.55g/l contre 13.6% chez les patients non opérés (p=0.5110). Conclusion : Les patients présentant une AOMI symptomatique sont insuffisamment traités, y compris les patients présentant le risque cardiovasculaire le plus élevé et les plus suivis. Objective: To describe, in patient with LEAD (lower extremity artery disease), the prevalence of patients who have received optimal medical treatment and meet LDLc (low density lipoprotein cholesterol) and triglyceride targets according to international recommendations and to compare these data between patients with a vascular surgery history of the lower limbs and those without a vascular surgery history. Methods: This is a single-center retrospective study including patients with symptomatic LEAD. Data were collected from January 2020 to May 2022 in patients who consulted for a walking test. Results: 225 patients were included from January 2020 to May 2022: 51.1% of patients received tritherapy, 12.4% of patients have an LDLc in the target <0.55g/l and 27.6% of patients reach the target <0.70 g/l. Regarding triglyceride thresholds, 64.4% are within the target thresholds <1.50g/l. In patients undergoing lower limb surgery, 52.9% benefited from optimal medical treatment versus 50.0% in non-operated patients (p=0.6687), 10.6% were within the target of LDLc <0.55g/l compared to 13.6% in non-operated patients (p=0.5110). Conclusion: Patients with symptomatic LEAD are undertreated, even in patients with the highest cardiovascular risk and the most follow-up.
