Le centre médical Louis Guilloux (CMLG) est un centre pluriprofessionnel situé à Rennes dédié aux migrants primo-arrivants. Il propose un premier bilan de santé avant d’adresser les patients dans le droit commun en santé, sans que l’effectivité de ce rôle de passerelle n’ait été démontré. L’objectif principal de cette étude est d’évaluer le taux de suivi en médecine générale ambulatoire de patients migrants un an après leur dernière consultation au CMLG. Matériels et méthodes : Il s’agit d’une étude épidémiologique descriptive rétrospective et monocentrique chez les patients ayant consulté la dernière fois au CMLG en 2019. La recherche de consommation de soins a été réalisée à partir du Système National des Données de Santé (SNDS) via une extraction par la Caisse Nationale d’Assurance Maladie (CNAM). Les données ont été traitées et anonymisées par la Caisse Primaire d’Assurance Maladie (CPAM) d’Ille-et-Vilaine. Résultats : 389 patients ont été inclus. 89 % de la population analysée avait consulté au moins un médecin généraliste dans l’année suivant son départ du CMLG. La fréquence moyenne était de 6,5 consultations par patient à un an contre 1,1 consultation chez les patients relevant du parcours de soins coordonnés avec un médecin traitant désigné. 57 % de la population éligible au parcours de soins coordonnés avait déclaré un médecin traitant. Le seul facteur socio-démographique significatif de non consultation à un an était la durée de suivi au CMLG : plus elle était longue, moins les patients consultaient en médecine générale (p = 0,0098). Conclusion : Les patients migrants initialement suivis au CMLG consultaient un an après leur départ plus souvent que la moyenne des patients en Bretagne et déclaraient moins souvent un médecin traitant que les patients français. The Louis Guilloux Medical Center (CMLG) is a multi-professional center located in Rennes dedicated to newly arrived migrants. It offers an initial health check-up before referring patients to common health law, without the effectiveness of this gateway having been demonstrated. The main objective of this study is to evaluate the follow-up rate in outpatient general practice of migrant patients one year after their last visit to the CMLG. Materials and methods: This is a retrospective, monocentric, descriptive epidemiological study in patients who last visited CMLG in 2019. The research of healthcare consumption was carried out from the National Health Data System (SNDS) via an extraction by the National Health Insurance Fund (CNAM). The data were processed and anonymised by the Primary Health Insurance Fund (CPAM) in Ille-et-Vilaine. Results: 389 patients were included. 89% of the population analysed had consulted at least one general practitioner in the year following their departure from CMLG. The average frequency was 6.5 consultations per patient at one year compared to 1.1 consultations in patients covered by the coordinated care pathway with a designated treating physician. 57% of the population eligible for the coordinated care pathway had declared a treating physician. The only significant socio-demographic factor of non-consultation at one year was the duration of follow-up at the CMLG: the longer it was, the fewer the patients consulted in general practice (p = 0.0098). Conclusion: The migrant patients initially followed at the CMLG consulted a year after their departure more often than the average patient in Bretagne and reported a treating physician less often than the French patients.
