L’impact pronostique du scanner thoracique d’admission dans cette situation reste encore à définir. Matériel et méthodes : Nous avons rétrospectivement inclus des patients atteint de PID, hospitalisés en réanimation pour IRA, entre janvier 2014 et octobre 2020. Nous avons analysé tous les scanners thoraciques d’admission, selon une échelle visuelle et une approche automatique. Nous avons également recueilli de façon longitudinale les données de ventilation mécanique. Le critère de jugement principal était la mortalité en réanimation. Résultats : La mortalité en réanimation était de 40%. Le seul facteur qui y était indépendamment associé était la réponse aux corticoïdes (OR 0,09, IC95% (0,01 – 0,41)). La présence de rayon de miel au scanner thoracique était associée à une moins bonne réponse à la corticothérapie (OR 0,25, IC95% (0,06 – 0,89). Chez les patients sous ventilation mécanique, une pression motrice plus élevée 7 jours après l’intubation était associée à un moins bon pronostic. Conclusion : La présence d’une fibrose au scanner thoracique n’est pas associée à un pronostic péjoratif en réanimation, et ce même chez les patients intubés. Il paraît donc licite d’envisager l’intubation oro-trachéale chez ces patients, ce d’autant plus que la corticosensibilité est fréquente et associée à un meilleur pronostic. Introduction: Acute respiratory failure (ARF) is frequent in interstitial lung disease (ILD) patients and sometimes require ICU admission or even invasive mechanical ventilation. The prognosis impact of chest CT-scan has not been evaluated in this situation. Materials and methods: We retrospectively included patients with ILD-associated ARF, admitted to the ICU of Rennes’ tertiary hospital between January 2014 and October 2020. We analyzed every admission chest CT-scan, based on a visual and automatic approach. We also collected mechanical ventilation data after intubation. The primary outcome was ICU mortality. Results: The ICU mortality rate was 40%. The only factor that was independently associated with ICU mortality was corticosteroid responsiveness (OR 0.09, 95% CI (0.01 – 0.41)). Patients with honeycomb on CT-scan had a lower response to corticosteroid (OR 0.25, 95% CI (0.06 – 0.89)). In mechanically ventilated patients, a higher driving pressure 7 days after intubation was associated with a poorer outcome. Conclusion: The presence of fibrosis indices on chest CT-scan isn’t associated with a poorer outcome, even in intubated patients. Therefore, it seems appropriate to consider orotracheal intubation in ILD patients. The corticosteroid responsiveness is frequent and associated with a better survival.
