Diabète insulinodépendant Mesure du glucose en continu Systèmes d'aide à la décision Patients--Satisfaction Systèmes de communication sans fil en médecine INTRODUCTION : Nous avons développé avec l’école Centrale-Supélec Rennes l’application IRTA pour l’aide à l’insulinothérapie au regard des données de la Mesure Continue du Glucose (MCG). Cette étude évalue l’impact d’IRTA sur l’équilibre glycémique et la satisfaction, via des questionnaires dédiés, des patients diabétiques de type 1 utilisant le FreeStyle Libre (FSL). METHODES : Les données de 64 patients suivis au CHU de Rennes et formés au FSL ont été analysées sur cette étude interventionnelle, prospective, monocentrique et en avant-après avec une phase sans IRTA, suivie d’une phase avec des conseils personnalisés. Le critère de jugement principal était le pourcentage de temps passé dans la cible glycémique 70-180mg/dL (Time In Range, TIR) sur le dernier mois de chaque période. RESULTATS : IRTA n’a pas montré d’efficacité sur l’amélioration de l’équilibre glycémique par les données directes et indirectes, y compris chez les plus grands utilisateurs. Les scores à nos questionnaires montrent une excellente satisfaction au FSL et une bonne satisfaction à IRTA. CONCLUSION : IRTA est peu bénéfique sur la glycémie chez des patients expérimentés et éduqués au diabète. Les outils d’aide à la décision (DSS) sont en plein essor. Des études supplémentaires sont à réaliser pour définir des sous-populations chez qui IRTA serait la plus pertinente. INTRODUCTION : We developed with Centrale-Supélec school the IRTA application to help insulin therapy from Continuous Glucose Monitoring (CGM) data. This study evaluates the impact of IRTA on glycemic control as well as the level of satisfaction, thanks to specific questionnaires, of type 1 diabetic patients using the FreeStyle Libre (FSL). METHODS : The data from 64 patients followed at the University Hospital of Rennes and trained to use the FSL have been analyzed in this interventional, prospective, monocentric and before-after study during a period without IRTA, followed by another with personalized advice. The primary endpoint was the percentage of time spent within the glycemic target 70-180mg/dL (Time In Range, TIR) over the last month of each period. RESULTS : IRTA did not show any efficacy in improving glycemic control by direct or indirect data, including among the highest users. The scores to the questionnaires we used show an excellent level of satisfaction with FSL and a good level of satisfaction with IRTA. CONCLUSION : IRTA is of little benefit on glycaemia in experienced and educated patients with diabetes. Decision support systems (DSS) are fast-growing. Additional studies are needed to define the subpopulations in which IRTA would be the most relevant.
