La transpiration compensatrice est le principal effet secondaire de la sympathectomie thoracique chirurgicale pour le traitement de l’hyperhidrose ou éreutophobie. La reconstruction du nerf sympathique en utilisant un greffon de nerf intercostal ou sural est une option chirurgicale qui permettrait de restaurer une chaîne sympathique fonctionnelle. Méthodes : Nous avons conduit une étude monocentrique et rétrospective sur nos résultats de la greffe de nerf sympathique. Tous les patients ayant bénéficié d’une greffe de nerf sympathique par vidéothoracoscopie ou robotique dans notre institution entre Septembre 2015 et Décembre 2021 ont été inclus soit un total de 14 patients. Le critère de jugement principal est l’évaluation de l’impact de cette intervention sur la qualité de vie des patients, cette évaluation a été réalisée au minimum 6 mois après l’intervention pour permettre une régénération du nerf optimale. Résultats : La localisation la plus fréquente dans notre étude de la transpiration compensatrice est le thorax, le dos et l’abdomen. Nous avons utilisé pour la transplantation un nerf intercostal dans 8 cas (57%) et un nerf sural dans 6 cas (43%). La durée médiane d’hospitalisation dans notre cohorte est de 2 jours (range ; 2-5). La moitié des patients a présenté une amélioration de leur qualité de vie après l’intervention. Conclusion : La transplantation de nerf sympathique par un greffon intercostal ou sural peut être une option chirurgicale dans le traitement de la transpiration compensatrice chez des patients très sélectionnés. Background : Primary hyperhidrosis is characterized by excessive sweating of face, palms, or axilla, occurring in 1-4,5% in population-based studies. Compensatory sweating is the main adverse event after thoracic sympathectomy for primary hyperhidrosis or erythrophobia. Sympathetic nerve reconstruction using an intercostal or sural nerve graft is a surgical option to restore a functional sympathetic chain pathway. Methods : We conducted a monocenter retrospective study. All Patients who underwent a thoracoscopic or robotic sympathetic nerve transplantation in our department between September 2015 and December 2021 were included. The primary outcome of this study is to evaluate the impact of this procedure in patients’ quality of life. The impact on quality of life was inquired with DLQI and HDSS validated dermatology questionnaires at least 6 months from the intervention to allow an optimal nerve regeneration. Results : The most common localization of compensatory sweating was chest, back and abdomen. Concerning the nerve graft, an intercostal graft was used in 8 cases (57%) and a sural graft in 6 cases (43%). The median hospital length of stay was 2 days (range; 2-5). In our cohort 1 patient present a postoperative hematoma with a reintervention for hemostasis. Three patients replied that the effect of nerve graft transplantation was “Excellent”, 4 responded “Mild”; 6 responded “Absent” and 1 responded “Worth”. Conclusion Sympathetic nerve transplantation with an intercostal or sural nerve graft may be an option to treat compensatory sweating in highly selected patients. Half of the patient in our study present an improvement in quality-of-life questionnaire that seems to be concordant with the other studies on the matter. However, the number of patients in this study is not sufficient for data analysis, other studies with larger sample of patients are required before any recommendation.
