Dans ce travail, nous avons, grâce à une approche transversale (transcriptomique, histologique et plasmatique), tenté de caractériser ces tumeurs et d’identifier des biomarqueurs pronostiques. Par une approche ciblée du microenvironnement tumoral (MT) de l’ADCP nous avons mis en évidence d’importantes altérations au niveau moléculaire. Nous avons notamment identifié, le rôle indépendant du MT dans les processus associés à l’agressivité et la progression du cancer. La protéine Stratifin, dont le gène a été identifié comme up-régulé dans le transcriptome du MT, a été confirmée comme surexprimée au niveau tissulaire dans le MT d’une cohorte monocentrique d’ADCP réséqués. Nous avons démontré que son expression tissulaire était associée de manière significative et indépendante à un mauvais pronostic. Ces résultats ont été confirmés dans une cohorte indépendante histologique multicentrique d’ADCP réséqués avec deux relectures de pathologistes. Au niveau plasmatique nous avons identifié que le caractère sécrété de la Stratifin et nous avons démontré qu’un niveau plasmatique élevé était associé un risque de récidive de l’ADCP. Pour finir nous avons comparé au niveau moléculaire l’ADCP et le CCD. A partir de tumeurs totales, nous avons démontré que le profil transcriptomique du CCD est plus proche de l’ADCP que du reste des tumeur biliaires. Ces résultats soutiennent hypothèse de l’existence d’une entité commune constitué par ces deux cancers. Notre travail apporte nouveau regard sur les tumeurs du carrefour bilio-pancréatique. L’étude du MT de l’ADCP a permet d’identifier un nouveau biomarqueur pronostic qui doit être confirmé dans un travail prospectif. Pancreatic ductal adenocarcinoma (PDAC) and distal cholangiocarcinoma (DCC) constitute a group of very similar digestive cancers with a very poor prognosis. In this work, we aimed to characterize these tumors and to identify prognostic biomarkers using a transversal approach (transcriptomic, histological and plasmatic). Through a specific approach of the tumor microenvironment (TM) of PDAC, we highlighted important alterations at the molecular level. In particular, we have identified the independent role of the TM in processes associated with cancer aggressiveness and progression. The Stratifin protein, whose gene was identified as upregulated in the TM transcriptome, was confirmed to be overexpressed at the tissue level in the TM of a monocentric cohort of resected PDACs. We demonstrated that its tissue expression was significantly and independently associated with poor prognosis. These results were confirmed in an independent histological multicenter cohort of resected PDAC with two pathologists' reviews. At the plasma level we identified that the secreted nature of stratifin and demonstrated that a high blood level was associated with a risk of PDAC recurrence. Finally we compared PDAC and DCC at the molecular level. Using total tumors, we demonstrated that the transcriptomic profile of CCD is closer to PDAC than to the rest of the biliary tumors. These results support the hypothesis of the existence of a common entity constituted by these two cancers. Our work provides a new insight into the tumors of the biliary-pancreatic junction. The study of the MT of the PDAC contributed to the identification of a new prognostic biomarker that needs to be confirmed in a prospective work.
