Nous nous sommes interrogé sur la toxicité de la population AJA qui a bénéficié d’une chimiothérapie à forte dose (HDC - high dose chemotherapy) dans l’essai Euro-E.W.I.N.G.99. Méthodes : Les patients à risque élevé étaient définis par une maladie localisée à risque élevé ou par des métastases pulmonaires ou pleurales. La chimiothérapie standard (VAI) et la HDC (BuMel) ont été randomisés. Nous avons analysé les toxicités sévères des patients français selon 3 groupes d’âge : < 15, 15-24, > 25 ans. Les toxicités de tout grade et l’intensité de dose étaient des points secondaires d'analyse. Résultats : Nous avons comparé les toxicités de 64 et 71 patients à haut risque traités par BuMel et VAI respectivement. Nous avons montré que HDC était faisable quel que soit l’âge des patients et que l’intensification de BuMel était aussi toxique que la chimiothérapie standard pour les AJA. Conclusion : Les patients à risque élevé devraient bénéficier de l’intensification, quel que soit leur âge. Cette étude a souligné l’importance de traiter ces patients dans un centre d’experts pour HDC avec des équipes formées. Introduction: Ewing sarcoma is a rare cancer affecting adolescents and young adults (AYA). All grade toxicities and dose intensity were second end-points. Results: We compared toxicities of 64 and 71 high-risk patients treated by BuMel and VAI respectively. We showed that HDC was feasible whatever the age of patients and that BuMel intensification was as toxic as standard chemotherapy for AYAs. Conclusion: High risk patients should benefit from intensification regardless of age.
