Le Transversalis Fascia Plane (TFP) bloc est une technique d’anesthésie loco-régionale qui pourrait avoir un intérêt dans la prise en charge analgésique de ces patients. Matériels et Méthodes : Cette étude monocentrique prospective randomisée en simple aveugle a inclus des patients opérés d’une chirurgie orthopédique de membre pour laquelle un prélèvement de greffon osseux iliaque antérieur était prévu. Le site opératoire principal était couvert par une anesthésie locorégionale et le protocole d’anesthésie et d’analgésie per- et post-opératoire étaient standardisés. Le groupe TFP bénéficiait de la réalisation d’un TFP bloc préopératoire, le groupe infiltration bénéficiait d’une infiltration d’anesthésiques locaux au niveau de la crête iliaque par le chirurgien. Le critère de jugement principal était la consommation de morphine des 48 premières heures. Les autres paramètres étudiés étaient les scores de douleur postopératoire précoce et à 3 mois. Résultats : 33 patients ont été inclus dans l’étude (16 dans groupe TFP et 17 dans groupe infiltration) entre mai 2015 et décembre 2018. La consommation de morphine (à 48 heures : 18.6 ± 30,1 contre 15.3 ± 24.3. p =0.99) et les scores de douleurs à la crête iliaque (à 24 heures : 1.7 ± 1.8 contre 2.8 ± 3. p =0.39) étaient comparables entre les 2 groupes à toutes les évaluations sans différence significative. Conclusion : Un bloc TFP ou une infiltration d'anesthésiques locaux permettent une analgésie postopératoire similaire lorsqu’ils sont accompagnés d’un protocole d’analgésie multimodale pour le prélèvement de greffon osseux iliaque. Introduction : Anterior iliac crest bone graft harvesting is commonly associated with high acute and chronic pain. Standard management of this complication includes an infusion of local anaesthesia at the donor site. Preemptive analgesia using the Transversalis Fascia Plane (TFP) block, a locoregional anesthesia procedure, could show benefit in this indication. Methods : This prospective single-center, randomized, single-blind study included patients planned for an orthopedic limb surgery associated with an anterior iliac crest bone harvesting. Per and post-operative anesthesia and analgesia protocols where standardized, including a locoregional anesthesia of the main surgical site. The TFP group had a TFP block performed before incision, and the infiltration group had a single shot local anesthesia infusion at the iliac crest performed by the surgeon at the end of the harvesting. Main outcome measurements were morphine consumption in the first 48 hours, and acute pain evaluations and a 3 months DN4 form where also analyzed.
