La mise en place du dispositif Oncoville en 2018, dans notre centre hospitalier a fait ressortir des résultats positifs avec 379 patients inclus, via une forte implication des médecins spécialistes et la volonté des pharmaciens hospitaliers d’intervenir dans la prise en charge médicamenteuse du patient. Le dispositif Oncoville a permis de renforcer le lien ville hôpital. L’implication des pharmaciens d’officine dans le suivi du patient sous anticancéreux oral s’est illustré par la réalisation de 230 consultations pharmaceutiques au cours des 3 ans de l’expérimentation. Ces résultats sont encourageants et afin de pérenniser ce parcours de soins coordonné ville-hôpital, l’établissement s’est engagé dans un article 51 Onco’Link Thérapies Orales. The rise of oral anticancer drugs in the treatment of cancer in France requires the coordination of city and hospital actors between to improve the patient's healtchcare pathway. The implementation of the Oncoville project in 2018, in our hospital center has shown positive results with 379 patients included through a strong enthusiasm from medical specialists, and the willingness of hospital pharmacists to be a part of the patient’s medication-related healthcare. The Oncoville project allowed it possible to strengthen the city-hospital link. The involvement of community pharmacists, who performed 230 pharmaceutical consultations, over the 3 years of the project, was important. With the Onco’Link project, for oral anticancer drugs, we can perpetuate the link created betwenn city’s health professionals and hospital, promising for the patient’s healthcare pathway.
