Matériel et méthodes : Étude rétrospective cas-témoin réalisée sur des patients greffés au CHU de Rennes entre janvier 2013 et décembre 2020. Les patients ayant reçu une ETP par le pharmacien ont été inclus dans le groupe intervention (n=139) et comparés à une cohorte de patient n’ayant pas reçu une ETP par le pharmacien (=131). Les concentrations minimales (Cmin) en tacrolimus dans les 3 mois suivant la sortie post-greffe ont été relevées puis comparées entre les deux groupes par analyse du coefficient de variation des Cmin. Résultats : Dans le groupe intervention, il y avait moins de patients présentant une variabilité élevée des Cmin (9% vs 13%) et plus de patients présentant une faible variabilité des Cmin (55.4% vs 44.2%). Conclusion : L'implication d'un pharmacien dans l'ETP après une transplantation rénale contribue à améliorer la variabilité intra-individuelle des Cmin et donc à améliorer l’adhésion thérapeutique du patient. Objective: To evaluate the impact of the integration of the pharmacist in a therapeutic education program for renal transplant patients on therapeutic adherence. Material and methods: Retrospective case-control study carried out on transplant patients at the University Hospital of Rennes between January 2013 and December 2020. Patients who received therapeutic patient education by the pharmacist were included in the intervention group (n=139) and compared with a cohort of patients who did not receive therapeutic patient education by the pharmacist (=131). Minimum tacrolimus concentrations (Cmin) within 3 months of post-transplant discharge were recorded and compared between the two groups by analysis of the coefficient of variation of Cmin. Results: In the intervention group, there were fewer patients with high Cmin variability (9% vs 13%) and more patients with low Cmin variability (55.4% vs 44.2%). Conclusion: Involvement of a pharmacist in therapeutic education of the patient after renal transplantation contributes to improve intra-individual variability of Cmin and thus to improve the therapeutic adherence of the patient.
