Insuffisance tricuspidienne Échocardiographie Cathétérisme cardiaque Hypertension artérielle pulmonaire‎ Contexte : L'échocardiographie transthoracique (ETT) semble ne pas être fiable pour l'estimation de la pression artérielle pulmonaire systolique (PAPs) chez les patients présentant une insuffisance tricuspidienne (IT) sévère. Cette étude visait à déterminer la fiabilité de l'é valuation de la PAPs par ETT dans cette population. Méthodes : Il s'agit d'une analyse monocentrique de 236 patients bénéficiant d’un cathétérisme cardiaque droit (KTD) et d’une ETT de manière concomitante. Le coefficient de concordance de Lin (CCL) et l'analyse de Bland-Altman (BAA) ont été utilisés pour comparer les valeurs de RHC et de TTE. Résultats : La concordance entre les deux procédures était bonne dans la population générale (LCC = 0,79 ; 95% limites d'agrément (LA) : 0,74 ; 0,84), mais plus faible chez les 78 patients (33%) avec IT sévères (LCC = 0,67 ; 95% LOA : 0.49 ; 0.8), avec une propension à une sous-estimation par l’ETT. L'altération de la fonction ventriculaire droite n'était pas associée à une sous-estimation, contrairement à une pression de l’oreillette droite (POD) élevée, qui a été associée à une sous-estimation par TTE d'environ 8 mmHg. La présence d’une « ventricularisation » de la POD (Odds-Ratio (OR) = 3,32 ; 95% Intervalle de Confiance (IC) : 1,02 ; 10,9 ; p =0,04) et une PAPs invasive ≥ 50 mmHg (OR = 2,63 ; 95 % IC : 1,35 ; 5,16 ; p < 0,01) étaient deux facteurs indépendants d'une mauvaise estimation de la PAPs en ETT. Conclusion : La présence d’une IT sévère conduisait à une sous-estimation par l’ETT, notamment si la courbe de POD était « ventricularisée » et en cas de PAPs invasive élevée. Background : Transthoracic Echocardiography (TTE) seems to be mistaken for the estimation of systolic Pulmonary Artery Pressure (sPAP) in patients with severe Tricuspid Regurgitation (TR). This study aimed to determine the reliability of the assessment of sPAP by TTE in this population. Methods : This is a monocentric analysis of 236 patients with concomitant Right Heart Catheterization (RHC) and TTE. Lin’s Concordance Coefficient (LCC) and Bland-Altman Analysis (BAA) were used to compare the values of RHC and TTE. Results : The correlation between the two procedures was good in the total population (LCC = 0.79; 95% limits of agreement (LOA) : 0. 74 ; 0.84), but weaker in the 78 patients (33%) with severe TR (LCC = 0.67; 95% LOA : 0.49 ; 0.8), with a propensity to an underestimation by TTE. Impaired RV function did not lead to an underestimation, contrary to an elevated Right Atrial Pressure (RAP), which was associated with an underestimation by TTE of about 8 mmHg. The presence of “C-V waves” sign on the RAP curves (Odds-Ratio (OR) = 3.32 ; 95% Confidence Interval (CI) : 1.02; 10.9 ; p =0.04) and invasive sPAPb≥ 50 mmHg (OR = 2.63 ; 95% CI : 1.35; 5.16 ; p < 0.01) were two independent predictors of a misestimation of sPAP by TTE. Conclusion : Severe TR led to an underestimation by TTE, especially if RAP curve shows a “C-V waves” sign, and in case of high invasive sPAP.
