Ses applications sont multiples, permettant le diagnostic, l’évaluation pronostique ou encore l’adaptation des thérapeutiques associées aux pathologies du système nerveux central (SNC). Nous avons évalué son utilisation en réanimation par rapport aux recommandations européennes au sein du service de Réanimation Chirurgicale du CHU de Rennes. Matériels et Méthodes : Il s’agit d’une étude observationnelle, rétrospective, monocentrique, ayant inclus 160 patients admis entre le 1er Mai et le 31 Décembre 2020 au sein de l’Unité de Soins Continus et de Réanimation Chirurgicale du CHU de Rennes. L’objectif princi- pal de l’étude était de comparer les pratiques locales avec les recommandations de l’Euro- pean Society of Intensive Care Medicine, dans l’hypothèse d’une sous-utilisation de l’EEG. Une analyse secondaire a été entreprise pour l’ensemble des tracés effectués sur cette pé- riode, concernant leurs indications, leurs modalités de demande, leurs résultats et leurs in- fluences sur la prise en charge des malades. Résultats : 160 patients ont été inclus, dont 69% d’hommes, avec un âge médian de 57ans. La mortalité dans cette cohorte s’élevait à 21%. Le motif d’hospitalisation était majoritairement d’origine neurologique (64%) (Traumatisme crânien 33%, hémorragie sous-arachnoïdienne 14%, accident vasculaire cérébral hémorragique 11% et ischémique 2%, état de mal épilep- tique 4%). Les recommandations étaient suivies pour 96% des patients admis pour hémorra- gies sous-arachnoïdiennes. A l’inverse, seulement 40% des indications étaient confirmées par un EEG concernant les pathologies extracrâniennes. 165 EEG ont été réalisés et 96 malades ont bénéficié d’au moins un enregistrement EEG. Seulement 67 demandes remplissaient des critères qualité satisfaisants. La plupart des explorations étaient effectuées pour cause de coma inexpliqué (47%). Aucun tracé n’était normal. Près de la moitié des examens étaient as- sociés à une modification de stratégie thérapeutique (44%). La réalisation d’un EEG était as- sociée significativement à des durées de séjour (médiane 14j [8-23] vs 8j [4-15]) et de ventila- tion prolongées (médiane 10j [4-18] vs 5j [2-12]). Conclusion : L’EEG revêt une place centrale dans le diagnostic et la prise en charge thérapeutique des patients de réanimation. Son utilisation n’est néanmoins pas optimale dans notre centre. Des pistes d’amélioration sont disponibles pour permettre de tirer pleinement profit d’une technique dont les indications sont amenées à s’étendre dans les années à venir. Abstract : Background: The electroencephalogram (EEG) allows the functional exploration of brain activity. Its applications are multiple, allowing the diagnosis, the prognosis evaluation or the adaptation of therapeutics associated with the pathologies of the central nervous system (CNS). We evaluated the particularities of its use in the Surgical Intensive Care Unit of the University Hospital of Rennes. Material and Methods: This is an observational, retrospective, single-center study that in- cluded 160 patients admitted between May 1 and December 31, 2020, to the High Depend- ency Unit and Surgical Intensive Care Unit of the University Hospital of Rennes. The main ob- jective of the study was to compare local practices with the recommendations of the European Society of Intensive Care Medicine, in the hypothesis of an under-use of EEG. A secondary analysis was undertaken for all of the EEG monitoring tracers during this period, including their indication, their request, their result and their influence on the patient management. Results: 160 patients were included, 69% were men, with a median age of 57 years. The mortality rate in this cohort was 21%. The reason for hospitalization was mainly neurological (64%) (Traumatic brain injury 33%, subarachnoid haemorrhage 14%, hemorrhagic stroke 11%, ischemic stroke 2%, and status epilepticus 4%). Recommendations were followed for 96% of SAH. Conversely, only 40% of the indications were followed for extracranial pathologies. 165 EEGs were performed and 96 patients received at least one. Only 67 requests met quality criteria. Most of the explorations were performed because of unexplained coma (47%). No tracings were normal. Almost half of the examinations were associated with a change in therapeutic strategy (44%). The implementation of an EEG was significantly associated with a prolonged length of stay and ventilation. Conclusions: EEG has a central place in the monitoring of ICU patients, directly impacting their therapeutic management. Nevertheless, its use is not optimal in our center. Improvements are available to take full advantage of a technique whose indications could be extended in the coming years.
