Il s’agit donc d’évaluer les critères diagnostics d’un score échographique pleuropulmonaire en comparaison au scanner thoracique chez les patients suspects de pneumopathie à COVID -19 ; puis évaluer le lien entre ce score échographique et le degré d’atteinte pulmonaire scanographique. Matériel et méthode Cette étude prospective observationnelle monocentrique, incluant 100 patients suspects d’infection à COVID-19 bénéficiant d’un scanner thoracique a été réalisée de mars 2020 à mai 2021 au SAU du CHU de Rennes. Un score échographique pleuropulmonaire a été établi et une analyse statistique afin d’établir un seuil de positivité ; puis une corrélation entre le score et le degré d’atteinte scanographique a été recherchée. Résultats Le seuil de positivité du score a été fixé à 10 pour un score maximal de 24. En prenant en compte ce résultat, les qualités métriques du score retrouvent une sensibilité à 48% [35.1%;61.3%], une spécificité à 93.18% [85.7%;100%] et une valeur prédictive positive à 90% [79.3%;100%]. Une corrélation positive avec un p < 0,0001 est établie en l’importance de l’atteinte scanographique pulmonaire et le score échographique. Conclusion L’échographie pleuropulmonaire, est un outil diagnostic et pronostic dans la pneumopathie à COVID-19, permettant d’orienter précocement les patients au SAU et de ce fait optimiser les flux, ainsi que détecter les patients à risque de dégradation respiratoire. Lung ultrasound (US) is becoming increasingly popular in the emergency department (ER) and therefore its application for evaluation of SARS-CoV2 lung injury needs to be developed. This study compared lung US to chest Computed Tomography scanner (chest CT) in patients with suspicion of COVID-19 pneumonia; then rate the link between the ultrasound score and the degree of pulmonary CT scan involvement Material and method We performed a prospective observational monocentric study between March 2020 and May 2021 in the ER department of the University Hospital of Rennes, France. One hundred patients with suspicion of COVID-19 infection requiring a CT scanner were included. A lung US score of gravity was created and we performed a statistical analysis in order to set a threshold of positivity value. Secondly we evaluated the correlation between our lung US score and the pulmonary lesions of the CT scanner. Results Our US lung score ranged from 0 to 24 and the threshold for positivity value of the lung US score was set at 10 to obtain a sensitivity of 48% [35.1%; 61.3%], a specificity of 93.18% [85.7%; 100%] and a positive predictive value of 90% [79.3%; 100%]. There was a positive correlation between the gravity of the pulmonary lesions in CT scanner and lung US (p<0.0001). Conclusion Lung US is useful as a diagnostic and prognostic tool in the early management of patients with COVID-19 pneumonia in the ER department.
