La survie avec DAVG n’a fait que s’améliorer, laissant apparaitre la survenue de Chirurgies Non Cardiaques (CNC) chez les porteurs de dispositif. Objectifs : Référencer les CNC et leur impact éventuel chez les porteurs de DAVG. Schéma d’étude : Analyse rétrospective de notre base de données locale prospective. Lieu : Chirurgie cardio-thoracique du CHU Rennes, entre janvier 1999 et juin 2021. Participants : Tous les patients adultes implantés d’un DAVG à flux continu. Était retenue comme Chirurgie Non Cardiaque, toute procédure nécessitant la présence d’un anesthésiste et non liée au DAVG ou un état d’insuffisance cardiaque. Résultats : Douze patients ont subi un total de 20 CNC parmi les 86 implantés de DAVG à flux continu. Les CNC concernaient principalement des porteurs en thérapie définitive, et dans 90% des cas au cours d’une hospitalisation différente que celle de l’implantation du DAVG. Le temps médian peropératoire passé avec une pression artérielle moyenne inférieure à 65 mmHg était de 15 (0-30) minutes. Douze (60%) des chirurgies ont nécessité une admission en soins intensifs au décours. La principale complication postopératoire était le saignement (15% des cas), et aucune thrombose n’a été déplorée. Un seul patient est décédé au cours de son hospitalisation avec CNC. La survie à 2 ans n’était pas significativement différente entre patients porteurs de DVAG opérés de CNC et les autres (log-rank p=0.4). Conclusion : Plus d’un patient sur sept porteurs de DAVG à flux continu a été opéré de CNC. La gestion anesthésique de ces patients reste très méconnue hors du bloc cardio-thoracique, mais l’on a constaté peu de complications postopératoires et l’absence de différence en terme de survie malgré la prise en charge pour CNC. Background: Left Ventricular Assist Devices (LVAD) have found an increasing place in the treatment of advanced heart failure, as a bridge to heart transplantation or a destination therapy. Patients’ survival continuously raised and led to non-cardiac surgeries (NCS) for the treatment of other diseases. Objectives: To investigate NCS and their impact among patients implanted with LVAD. Design: A retrospective analysis of prospectively collected data in our local database. Setting: The cardiothoracic surgery ward of Rennes University Hospital, from January 1999 to June 2021. Population: All adult patients implanted with durable Continuous Flow LVAD (CF-LVAD). Non-Cardiac Surgery was defined as any surgical procedure requiring an anesthetist and unrelated to VAD or heart failure. Main Results: Eighty-six patients were implanted with CF-LVAD during the study period, of whom 12 (14%) underwent a total of 20 NCS. NCS were mainly performed in patients implanted as a destination therapy, and mostly (90%) during a different hospitalization than implantation. Median time with mean arterial pressure under 65 mmHg during NCS was 15 (0-30) minutes. Twelve (60%) surgeries led to admission in intensive care unit. The main postoperative complication was bleeding (15% of NCS), and no thrombosis was found. One patient died after NCS. Cumulative 2-year survival was not different between CF-LVAD patients operated or not of a NCS (log-rank p=0.4). Conclusion: More than one CF-LVAD patient over seven was operated of NCS, and the probability for an anesthetist to deal with these patients constantly increases. Although per-operative management could be fearing, these patients had few complications and we could not demonstrate a difference in survival, as compared with non-operated ones.
