La sécurité était une priorité du protocole d'anesthésie sans opioïdes. Par conséquent, cette étude a été réalisée pour déterminer les facteurs de risque associés à la bradycardie peropératoire lors d'une anesthésie sans opioïdes utilisant la dexmedetomidine. Méthodes : Le protocole anesthésique des patients subissant une chirurgie non-cardiaque intermédiaire ou majeure était standardisé. Selon leur groupe de randomisation, les patients ont reçu de la Dexmédétomidine ou du rémifentanyl. Deux groupes ont été établis (groupe bradycardie et groupe non-bradycardie) selon qu'ils aient présenté ou non un épisode de bradycardie. Les facteurs de risque de bradycardie ont été mis en évidence par une analyse de régression logistique. Résultats : Nous avons identifié 3 facteurs de risque indépendants de bradycardie peropératoire : recevoir de la Dexmédétomidine en peropératoire (OR 3,22 (IC95% 1,42 à 7,27 - p-value = 0,005)), une fréquence cardiaque préopératoire inférieure à 82bpm (OR 4,56 (IC95% 1,34 à 15,5 - p-value = 0,02)) et une dose cumulée de Cisatracurium peropératoire inférieure à 85mg (OR 2,76 (IC95% 1,08 à 7,06 – p-valeur=0,03)) Conclusion : L'association de ces risques expose le patient à un risque élevé de bradycardie. Une sélection préopératoire des patients en prêtant une attention spécifique à ces facteurs de risque permettrait aux patients de bénéficier d'une anesthésie sans opioïde sans bradycardie. BACKGROUND: The POFA study recently reported that patients receiving opioid-free anaesthesia with dexmedetomidine experienced more intraoperative severe bradycardia and postoperative hypoxemia with a longer time to extubation and a longer PACU stay. Following the prematurely termination of the POFA study because of 5 episodes of profound bradycardia, we aimed to determine the risk factors associated with intraoperative bradycardia during opioid-free anaesthesia using dexmedetomidine. OBJECTIVE To study the risk factors associated with intraoperative bradycardia during opioid-free anaesthesia using dexmedetomidine. DESIGN Post-hoc analysis of a randomized controlled trial. SETTING Ten French hospitals from March 2017 to January 2019. PARTCIPANTS Patients scheduled for intermediate or major non-cardiac surgery. INTERVENTION A standardized balanced anaesthesia protocol was applied with dexmedetomidine or remifentanil according to randomization. MAIN OUTCOME AND MEASURE: The primary endpoint was intraoperative bradycardia. RESULTS Of the 314 patients included in the analysis, 34 presented a bradycardia episode. We identified 3 independent risk factors of intraoperative bradycardia: intraoperative dexmedetomidine (OR 3.22 (IC95% 1.42 to 7.27 – p-value = 0.005)), pre-operative heart rate lower than 82bpm (OR 4.56 (IC95% 1.34 to 15.5 – p-value = 0.02)) and cumulative intraoperative cisatracurium dose lower than 85mg (OR 2.76 (IC95% 1.08 to 7.06 – p-value=0.03)) CONCLUSION Patients presenting the association of these risks are at high risk of bradycardia. A preoperative selection of the patients associated with specific attention to these intraoperative risk factors would allow patients to safely benefit from opioid-free anaesthesia.
