     <dcterms:abstract xml:lang="fr">La prévalence des maladies chroniques est en augmentation et l’insuffisance rénale chronique n’y échappe pas. Cette dernière évoluant de façon sournoise et silencieuse, son diagnostic est trop souvent tardif et sa prévalence au sein de la population est sous-évaluée. En France, le nombre de personnes atteintes d’une MRC est estimé à près de 5,7 millions. L’IRCT affecte une proportion croissante de la population : en 2018, il y avait 89 692 patients prévalents bénéficiant d’un traitement de suppléance. Ainsi, la détection précoce de la MRC doit être une priorité. Les traitements par suppléance (dialyse, greffe) ont transformé la prise en charge de la maladie depuis plus de 50 ans. Devant l’hétérogénéité des traitements et de la complexité de la maladie et des actions correctrices, il est essentiel de considérer les patients comme des acteurs de leur prise en charge. Cette dernière s’accompagne alors d’une éducation thérapeutique des patients, intégrée aux soins. Cette thèse a pour vocation de faire un état des lieux de l’IRC et d’aborder la mise en place des programmes d’ETP. En effet, l’objectif de ce travail est de considérer les différents objectifs et mesures correctrices des démarches personnalisées dans le but d’établir des recommandations en vue de son amélioration. La première partie est consacrée à l’approche scientifique du rein : épidémiologie, maladies, mécanismes d’action, diagnostic et prise en charge. La seconde partie quant à elle tente de répondre à la problématique suivante : en quoi la mise en place d’un programme d’éducation thérapeutique peut-elle être bénéfique dans le cas des maladies rénales chroniques ?</dcterms:abstract>
     <dcterms:abstract xml:lang="en">The prevalence of chronic disease is increasing and chronic kidney failure is no exception. Chronic kidney disease evolves in an insidious and silent way, its diagnosis is too often late and its prevalence in the population is underestimated. In France, the number of people suffering from chronic kidney disease is estimated at nearly 5.7 million. Chronic kidney disease affects a growing proportion of the population: in 2018, there were 89,692 prevalent patients receiving replacement therapy. Therefore, early detection of chronic kidney disease must be a priority. Replacement therapy (dialysis, transplantation) has transformed the management of the disease for more than 50 years. Faced with the heterogeneity of treatments and the complexity of the disease and corrective actions, it is essential to consider patients as actors in their management. The latter is therefore accompanied by therapeutic patient education, integrated into the care process. The aim of this thesis is to take stock of the current state of chronic renal failure and to address the implementation of therapeutic patient education programs. Indeed, the objective of this work is to consider the different objectives and corrective measures of personalized approaches in order to establish recommendations for its improvement. The first part is devoted to the scientific approach of the kidney: epidemiology, diseases, mechanisms of action, diagnosis and management. The second part attempts to answer the following question: how can the implementation of a therapeutic education program be beneficial in the case of chronic kidney disease?</dcterms:abstract>
