L’objectif de notre étude était d’évaluer le pronostic des patients BTB et d’identifier les prédicteurs de mortalité post opératoire. Matériel et Méthodes : Cette étude a été menée dans 19 centres entre 2006 et 2016, seuls les patients en choc cardiogénique au moment de l’implantation du LVAD ont été analysés. Les patients ont été divisés en 2 groupes : ceux qui étaient sous TCS au moment de l’implantation du LVAD (=stratégie BTB) et ceux qui n’ont pas eu de BTB. Le critère de jugement principal était la mortalité à 30 jours. Résultats : 349 patients étaient en choc cardiogénique avec 173 patients et 176 patients dans le groupe BTB et non BTB, respectivement. Parmi cette population 68 (39.3%), 18 (10.4%) et 15 (8.7%) patients étaient sous ECMO VA, Impella, et Ballon de Contre pulsion Intra Aortique seuls, et 72 (20.6%) étaient sous TCS multiples. Les patients en BTB présentaient un taux de mortalité significativement supérieure à 30 jours (p=0.041) avec 37 (21.4%) décès versus 21 (11.9%) dans le groupe sans BTB. L’analyse multivariée retrouvait 3 prédicteurs indépendants de mortalité : la chirurgie combinée au LVAD, l’IMC, la durée de l’insuffisance cardiaque, et un prédicteur indépendant de survie qui était l’indication en bridge to transplantation. (BTT). Le groupe BTB avait une durée d’hospitalisation en réanimation significativement plus longue, (p<0.001), avec une durée de ventilation mécanique deux fois plus longue (p<0.001), mais les complications post opératoires n’étaient pas significativement différents dans les 2 groupes. Conclusion : La stratégie de BTB est associée à une mortalité plus importante. Les prédicteurs de mortalité dans ce groupe sont la durée de l’insuffisance cardiaque, la chirurgie combinée au LVAD, un IMC plus élevé. We aimed at analyzing the outcome of BTB patients and identify predictors of post-operative mortality. Methods: This study was conducted in 19 centers between 2006 and 2016 and only those in cardiogenic shock at the time of LVAD implantation were analyzed. Patients were divided in two groups: those under temporary circulatory support (TCS) at the time of LVAD implantation (=BTB strategy), and those who did not undergo a bridging strategy. The primary endpoint was 30 day mortality. Results: A total of 349 patients were in cardiogenic shock, 173 and 176 patients in the BTB and non BTB group, respectively. Among this population, 68 (39.3%), 18 (10.4%) and 15 (8.7%), patients were under VA-ECMO, Impella and IABP support alone, and 72 (20.6%) patients were under multiple TCS support. BTB patients presented significantly higher mortality at 30 days (p=0.041) with 37 (21.4%) deaths versus 21 (11.9%) in the non BTB group. Multivariate analysis identified 3 independent predictors of mortality: combined surgery with LVAD, body mass index, heart failure duration, and one independent predictor of survival which was bridge to transplantation (BTT) indication. The BTB group had a significantly longer intensive care unit duration stay (p<0.001), with two-fold duration of mechanical ventilation time (p<0.001), but post-operative complications were not significantly different in both groups. Conclusion: BTB strategy is associated with a higher mortality. Predictors of mortality in the BTB group are duration of HF, combined surgery with LVAD, higher BMI.
