Méthodes : Etude multicentrique, prospective et non interventionnelle incluant des patients atteints d’ABD, transfusés ou non, entre octobre 2017 et août 2019. Les données cliniques, la NFS et les réticulocytes ont été analysés. Le fer sérique, la transferrine, le coefficient de saturation de la transferrine, la ferritine, l’hepcidine, la CRP, le fer sérique non lié à la transferrine (NTBI) et ERFE ont été dosés. Résultats : 31 patients analysés, 17 étaient transfusés. Le fer sérique, la ferritine et le coefficient de saturation de la transferrine, l’hepcidine étaient plus élevés chez les patients transfusés, la transferrine plus basse. L’érythroferrone et le rapport ERFE/hepcidine ne différaient pas entre les 2 groupes. Conclusion : La surcharge en fer chez les patients ABD dépend de la gravité de la maladie et des traitements. Il existe une reprise de l’érythropoïèse chez tous les patients (sécrétion d’érythroferrone). Dans l’ABD, l’hepcidine répond plus à la surcharge en fer qu’à l’érythropoïèse. Iron metabolism, iron overload assessment and the role of erythroferrone (ERFE) in Diamond-Blackfan anemia (DBA) is not well known. Methods: Multicentric, prospective and non-interventional study including patients with DBA transfused or not, between October 2017 and August 2019. Clinical data, full blood cell count and reticulocytes were analyzed. Serum iron, transferrin, transferrin saturation, ferritin, hepcidin, CRP, Non-Transferrin Bound Iron and ERFE were measured. Results: 31 patients analyzed, 17 were transfused. Serum iron, ferritin, transferrin saturation and hepcidin were higher in transfused patients, transferrin was lower in transfused patients. Erythroferrone and the ERFE / hepcidin ratio did not differ between the 2 groups. Conclusion: Iron overload in DBA patients depends on the severity of the disease and the treatments. There is a recovery of erythropoiesis in all patients (secretion of erythroferrone). In DBA, hepcidin responds more to iron overload than to erythropoiesis.
